Determining the prognosis of periodontally diseased molars using the Miller-McEntire Periodontal Prognostic Scoring Index : a prospective study
Abstract:
Background: This two-year prospective survival analysis study aimed to determine the periodontal prognosis of diseased molars using the Miller-McEntire Periodontal Prognostic Scoring Index. Methods: 1032 molars were evaluated on 129 patients (75 males & 54 females) with moderate to severe chronic periodontitis. Out of these 129 patients included 200 molars in 25 patients with diabetes mellitus and 200 molars in 25 patients who are smokers. The parameters that were evaluated include age, probing depth, mobility, furcation involvement, smoking, molar type and periodontal attachment level. The purpose of this study is to prospectively evaluate and establish the prognosis of molar teeth based on the Miller-McEntire Periodontal Prognostic Scoring Index, over a period of 2 years post-therapy. As per the Miller-Mc Entire Periodontal Prognostic Scoring Index, the sum of all the scores for all the parameters was used to determine the prognosis score for each molar. All patients were evaluated at baseline and two years post-treatment. Results: The results of the current study suggest that the following prognostic factors namely smoking (HR=6.3) diabetes (HR=1.1), furcation involvement (HR=0.214), molar type (HR=0.253) and age (HR=0.056) were clinically significant at the end of two-year post-therapy from baseline. A total of 9(0.87 %) teeth were extracted out of 1032 teeth with a mean of 0.00872 for the tooth lost during two years post -treatment. Conclusion: The findings of the current study has established a new scoring for the factor diabetes and at the same time confirms the results of the original study by Miller et al. The factors smoking, diabetes, furcation involvement, molar type and age contribute majorly in the prognosis of the molar survival. However, a study with a larger sample size and longer follow-up prospectively would be required in the future to affirm the findings of this study.
