A scoping review of prosthetic interventions for transtibial bilateral and quadrilateral amputees
Abstract:
The Prosthetics world is increasing gradually each year. In the present time, lower limb amputation surgery and prosthetics have much improved, even though we still have very limited research regarding prosthetic treatments and outcomes on bilateral and quadrilateral amputees. Therefore, the purpose of this research was to perform a Systematic Review in the method of a Scoping Review to evaluate the evidence available to assist prosthetic clinical practice for transtibial bilateral and quadrilateral amputees regarding prosthetic management in 4 areas of interest: socket, suspensions and interface, feet and ankles and terminal device. Following some keywords, a systematic literature search was performed. Out of 206 studies, 16 were selected for review and evaluated according to the 13 criteria previously outlined. 6 studies scored at a level of significance of A, 9 studies at level B, and 1 study at level C. Three reviewers (MM, GG and IS) independently analyzed and scored the studies. In the reviewed articles, some evidence suggests that proper prosthetic treatment can improve patient's ADL. Bioenergetics and gait parameters are improved upon proper prosthetic management. Most of the prosthetic user reject wearing their prosthesis due to poor fit, poor biomechanics and reduced control. Suitable ankle-foot prosthesis can decrease the metabolic cost of walking., proper prosthetic intervention with sport prosthesis could enable sports performance of quadruple amputee. Prosthetic ankle-foot units were found to improve on walking speed. Ankle-foot prosthesis and Torsion Adapter provides additional motion in the sagittal, frontal, and transverse planes. The literature indicates that bilateral lower limb amputees can opt for cuff type prosthetic suspensions, gel locking liner or supracondylar suspensions. A figure of 9 harness or cross back strap on the upper limb harnessing can help the user don the upper prostheses and maintain suspension. Prosthetic socket serves to maintain the residual limb inside of the socket and overall prosthesis function. Upper limb sockets for quadrilateral amputee are typically rigid socket with flexible inner socket. The lower bilateral amputee necessitates a rigid outer socket. The patella tendon bearing (PTB) or total surface bearing (TSB) socket is typically used. Outcome Measures: A successful dynamic alignment can increase in symmetry in all levels. Intense physical therapy, proper prosthetic fitting with postoperative training can enhance mobility. Male gender is positively predicted of prosthetic fitting. Bilateral amputations, increasing age and Diabetes mellitus are negative predictors of prosthetic fitting. Closed-chain exercise and preprosthetic training may successfully becoming a community ambulator from novice ambulator. The low-cost and durable materials are sufficient to create upper-limb devices to meet functional needs and many other tasks.
