Cultural competence and cognitive behavioral therapy for drug addiction rehabilitation in the three Southern border provinces of Thailand

Abstract: Cognitive Behavioral Therapy (CBT) is widely applied in drug treatment facilities. However, few studies utilizing cultural competence principles have been conducted in Thailand to determine whether clients' responsiveness to CBT is affected by their cultural backgrounds. Accordingly, the objectives of this study were to investigate therapists' and clients' experience of CBT and to propose a model of cultural competency and cognitive behavioral therapy (CCBT) for drug addiction rehabilitation in the southern border provinces of Thailand. A mixed methods design built on cognitive conceptualization and the Ecological Validity Model (EVM) was conducted. The study found that clients were satisfied and willing to participate in a CBT conducted by a culturally sensitive therapist, particularly a therapist who practiced the same religion and spoke the same language. Other outcomes of the study were in several areas. Firstly, clients reported that no activity involving the exploration of deeper dimensions of life problems influenced sustained drug use. Therapists meanwhile reported their experience of CBT in relation to the cultural dimension: considering EVM dimensions, the CBT model was responsive to clients' cultural backgrounds predominantly in the areas of language, context, and personal dimensions. Additionally, the dimensions facilitating the use of cultural dimension in CBT to suit clients' backgrounds were found to consist of concepts and contents. It is suggested that the CBT structure and learning manuals should be revised to match clients' cultural values; furthermore, the term 'negative thought' should be changed. Moreover, therapists who conceptualize drug use behaviors based on the moral model have discovered that their judging behaviors have influenced their attitudes and style of therapeutic relationship. This study recommends that CCBT for drug addiction rehabilitation in the southern border of Thailand should consider redesigning CBT activities and skills in relation to the eight dimensions of EVM. Furthermore, the model content should be aligned with Islamic principles.

Abstract: รูปแบบการบำบัดแบบการปรับเปลี่ยนความคิดและพฤติกรรม (Cognitive Behavioural Therapy: CBT) ได้นำมาใช้อย่างแพร่หลายในสถานบำบัดยาเสพติด อย่างไรก็ตามพบการขาดแคลนการศึกษาวิจัยในประเทศไทยที่เกี่ยวข้องกับประสบการณ์การใช้ CBT ว่าสอดคล้องกับมิติวัฒนธรรมของผู้รับบริการมากน้อยอย่างไร โดยอิงแนวคิดทฤษฎี สมรรถนะทางวัฒนธรรม การศึกษานี้จึงมีวัตถุประสงค์เพื่อศึกษาประสบการณ์ของผู้บำบัดและผู้รับการบำบัดที่มีต่อการบำบัดตามแนวคิด CBT และนำเสนอโมเดลสมรรถนะทางวัฒนธรรมของการบำบัดทางความคิดและพฤติกรรมสำหรับการฟื้นฟูผู้ติดสารเสพติดในบริบทพื้นที่สามจังหวัดชายแดนใต้สุด ออกแบบการศึกษาแบบผสมผสานบนฐานของแบบจำลองความคิดและแนวคิด Ecological Validity Model (EVM) ผลการศึกษาพบว่า ประสบการณ์ของผู้รับการบำบัดที่มีต่อ CBT พึงพอใจและเต็มใจเข้าร่วมการบำบัดกับผู้บำบัดที่มีความไวทางวัฒนธรรม โดยเฉพาะผู้บำบัดที่นับถือศาสนาเดียวกันและใช้ภาษาเดียวกัน และให้ข้อสังเกตว่า ไม่พบกิจกรรมที่ลงลึกไปถึงความซับซ้อนของปัญหาที่เกิดขึ้นในชีวิตที่ส่งผลให้ยังใช้สารเสพติด ส่วนประสบการณ์ของผู้บำบัดในมิติวัฒนธรรมกับการบำบัด CBT นั้น พบว่ารูปแบบ CBT ที่สอดคล้องกับวัฒนธรรมของผู้รับการบำบัด เมื่อพิจารณากับองค์ประกอบที่ระบุไว้ใน EVM นั้น องค์ประกอบที่พบว่ามีปรากฏชัดเจน คือ ด้านภาษา ด้านบริบทบางประการ และด้านบุคคล ส่วนมิติที่ผู้บำบัดตระหนักว่า เห็นโอกาสในการใช้แนวคิดวัฒนธรรม ปรับเนื้อหากิจกรรม CBT ให้เข้ากับวัฒนธรรมของผู้รับการบำบัด ได้แก่ มิติ Concepts และ Content โดยเสนอแนะว่า ควรมีการปรับชุดความรู้และโครงสร้างของ CBT ให้เข้ากับ Cultural value และ การปรับคำว่าความคิดเชิงลบ นอกจากนั้นพบว่าผู้บำบัดที่มีฐานความคิดคุณธรรมในการเข้าใจพฤติกรรมเสพติด ตั้งข้อสังเกตว่าตนมักจะวิพากษ์พฤติกรรมเสพติด ซึ่งส่งผลต่อทัศนคติและสัมพันธภาพในการบำบัดรักษา ข้อเสนอโมเดลสมรรถนะทางวัฒนธรรมของการบำบัดทางความคิดและพฤติกรรมสำหรับการฟื้นฟูผู้ติดสารเสพติดในบริบทพื้นที่สามจังหวัดชายแดนใต้สุดควรคำนึงถึงการปรับกิจกรรมและทักษะต่างๆ ของรูปแบบ CBT ให้เข้ากับ ทั้ง 8 องค์ประกอบของ แนวคิด EVM การปรับเนื้อหาทักษะ CBT บางประการที่สอดคล้องตามหลักการอิสลาม
