Abstract: Background: Acne scars have been known to have a profound negative impact on quality of life psychologically and sociably. Multiple treatments have been developed to cure acne scars but all procedures may improve skin appearance but cannot establish a scar-free skin. Subcision and hyaluronic filler injection have been used to treat acne scars with different mechanisms. Objective: To compare effectiveness between subcision with combination of HA filler injection and subcision alone for treatment in atrophic acne scars. Methods: Seventeen subjects diagnosed with atrophic acne scars received scar subcision on both sides of the face and randomly received hyaluronic filler injection on one side of the face and followed up for 3 months. Assessments were evaluated by 3 blinded dermatologists. Results: Subcision with combination of HA filler significantly improved atrophic acne scar severity better than subcision alone after 8th week of the treatment (p=0.045) and even better after 12th week (p=0.033). Both of the treatment significantly improved the atrophic acne scars. Subcision with combination of HA filler significantly improved atrophic acne scar severity. The mean score of quantitative Goodman and Baron’s global scarring grading scale for the subcision with filler side were 6.12±4.27 before treatment and improved to 4.78±3.78 (p=0.001) at 8th week and 4.27±3.32 (p=0.001) at 12th week after treatment. Subcision alone also significantly improved atrophic acne scar. The Mean score of quantitative Goodman and Baron’s global scarring grading scale for subcision alone side were 5.88±3.33 before treatment them improved to 5.37±3.91 (p=0.025) at 8th week and 5.12±3.88 (p=0.009) at 12th week after treatment. Conclusion: The study has shown that Subcision with combination of HA filler significantly improved atrophic acne scar. Both of treatment were significantly improved the atrophic acne scars. Combination of the treatment yields more mechanism to improve the scar, resulted in more scar types with different underlying pathology improvement. Conclusion: Based on the results, 5% Niacinamide cream can be used as an effective, safe and promising therapeutic agent for infraorbital hyperpigmentation.
