Determinants of inconsistent condom use among male migrant workers in the coastal area of Thanbyuzayat Township, Mon State, Myanmar

Abstract: Migrants have been considered at risk of acquiring HIV infection as bridging populations along their migratory route. Consistent condom use is considered as an effective strategy in protecting against transmission of (STI), including HIV. Although social cognitive theory (SCT) is one of the most widely used models of sexual transmission risk behaviors, there is little study based on SCT with inconsistent condom use among migrant workers in Asian countries including Myanmar. The objective of this study is to explore prevalence and determinants of inconsistent condom use in last 12 months and at last sex among male migrant workers in the coastal area of Thanbyuzayat township, Mon state, Myanmar. A community based cross-sectional study was conducted in April 2017, and a total of 420 male migrant workers aged between 18-49 years who experienced sex within last 12 months were selected by two stage sampling techniques. The data were collected by a face-to-face interview with developed structured questionnaires based on SCT. The prevalence of inconsistent condom use in last 12 months and condom use at last sex were 92.6% and 78.8% respectively. Older age groups [35-49 years (AOR=6.12, 95% CI (1.70-22.04) and 25-34 years (AOR=2.73,95%CI (1.04-7.17)], those from Ayyarwady as an origin [(AOR=4.23, 95% CI (1.04-17.18)], currently married or living with partner [(AOR=2.61, 95% CI (1.03-6.61)], condom use decision by partner [(AOR=0.23, 95% CI (0.06-0.86)], positive hedonistic outcomes [(AOR=2.74, 95% CI (1.16-6.50)] were associated with inconsistent condom use in last 12 months. Factors associated with no condom use at last sex were older age group [35-49 years (AOR=2.67, 95% CI (1.20-5.78)], labor [(AOR=2.16, 95% CI (1.06-4.41)] and self-employment [(AOR=2.69, 95% CI (1.16-6.22)], currently married or living with partner [(AOR=2.66, 95% CI (1.38-5.11)], poor condom accessibility [(AOR=3.36, 95%CI (1.62-6.98)], high social support [(AOR=5.40, 95% CI (2.39-10.16)], good knowledge [(AOR=2.39, 95% CI (1.22-5.05)] and negative partner expected outcomes [(AOR=1.81, 95% CI (0.98-3.29)]. The key finding suggested that provision of HIV service packages to migrant cluster including condom distribution especially on married couples should be implemented.
