The development and psychometric properties of the risks of depression relapse scale for adult patients with major depressive disorder
Abstract:
Major Depressive Disorder (MDD) is an illness with incidence in populations worldwide in every gender and age group. Depression is a severe and chronic mental health problem characterized by frequent relapses to depression. Patients with (MDD) require continual treatment and follow-up. The same is true in Thailand where there are numerous instruments for measuring and evaluating or screening for depression or MDD, because the country is beginning to give importance to the issue with the development of a surveillance system for depression in various areas nationwide. Nevertheless, no instruments have been found for assessing the level of risk for depression relapse for adults patients with MDD in Thailand and developed based on the contexts of Thai culture, worship of the Buddhist religion with its influence on Thai lifestyles, society and traditional Thai beliefs. The present study is mixed methods research in exploratory sequential form divided into two phases. Phase 1 was the qualitative phase conducted by using the biopsychosocial model of Garcia-Toro & Aguirre (2007), which was the basis for designing guidelines for obtaining qualitative data. Data were collected on experiences with perceived risks for depression relapse in adult patients with MDD. Phase 2 was the quantitative phase in which the data findings obtained in Phase 1 were used to develop the components of the instrument for assessing risks for depression relapse based on the process of instrument formation of DeVellis (2012). Then the psychological properties of the instrument for assessing the risk for depression relapse among adult patients with MDD were tested. This instrument passed content validity testing by 7 experts. According to the findings, the content validity index was rather high (CVI = .86). The instrument also passed testing of the ease of comprehension of the questions among 5 adult patients with MDD. After the testing of its psychological properties, the instrument was composed of 20 questions. The instrument was examined for psychological properties by collecting data from 515 patients with major depressive disorder at the mental health and psychiatric clinics in the out-patient departments of hospitals in Health Area 4. Structural validity was tested by analyzing the survey components with principal axis factoring. The principal axis factoring had a factor weight of over .30, and the results of the confirmatory factor analysis revealed the form for assessing risks for depression relapse to be composed of the following three factors: 1) Negative Psychosocial Cultural Factors ; 2) Negative Psychosocial Cultural Factors and 3) Coping Behavioral Factors ; 54.86% of the variance in these findings can be explained. The results of the confirmatory factor analysis revealed the structure of the risk factors for depression relapse corresponded with the evidence-based practice within acceptable criteria (χ2=150.556, df = 124 (p>.001), RMSEA=.027, and SRMR =.049. The reliability values for the internal consistency of this instrument (Cronbachs alpha coefficients) ranged from .574 to .658 for individual factors, while the reliability of the entire instrument was .899. In addition, the distribution properties between the group of patients at risk for depression relapse and the group with no risks were tested with t-test with statically significant difference (p<.05) This study revealed that some biological factors needed to be eliminated, because the factor values could not be tested. Thus, the assessment form for risks of depression relapse contained only three factors as stated above. The reliability of the instrument was good for internal consistency, content validity and structural validity. The instrument can also be useful in assessing the risks for relapse of depression in adult patients with MDD in the Thai context
