Abstract: การศึกษานี้มีวัตถุประสงค์เพื่อวิเคราะห์ต้นทุนต่อหน่วยบริการทันตกรรมแยกตามรายการบริการของ โรงพยาบาลส่งเสริมสุขภาพตำบลสันมะเค็ด จังหวัดเชียงราย การให้บริการทันตกรรมได้แก่ การถอนฟัน, ขูดหินปูน, การอุดฟันด้วยวัสดุอมัลกัม 1 ด้าน, การอุดฟันด้วยวัสดุอมัลกัม 2 ด้าน, การอุดฟันด้วยวัสดุสีเหมือนฟัน 1 ด้าน, และการอุดฟันด้วยวัสดุสีเหมือนฟัน 2 ด้าน การศึกษานี้เป็นการศึกษาย้อนหลัง เก็บข้อมูล ตั้งแต่เดือนตุลาคม 2559 ถึงเดือนกันยายน 2560 โดยรวบรวมข้อมูลจากโปรแกรม JHCIS, การเบิกจ่ายวัสดุ, ค่าตอบแทนและสวัสดิการของทันตาภิบาล ทะเบียนครุภัณฑ์และสิ่งก่อสร้างปีงบประมาณ พ.ศ. 2560 ข้อมูลต้นทุนแยกเป็นต้นทุนค่าแรง ต้นทุนค่าวัสดุและต้นทุนค่าเสื่อม การวิเคราะห์ต้นทุนใช้การวิเคราะห์แบบต้นทุนฐานกิจกรรม ผลการศึกษาพบว่า ต้นทุนต่อหน่วยทันตกรรมของการให้บริการในโรงพยาบาลส่งเสริมสุขภาพตำบลต่ำกว่าค่ารักษาที่กำหนดไว้ของกรมบัญชีกลาง โดยต้นทุนต่อหน่วย ของงานถอนฟันเท่ากับ 137.05 บาท/ครั้ง งานขูดหินน้ำลายเท่ากับ 149.93 บาท/ครั้ง งานอุดฟันด้วยอมัลกัม 1 ด้านเท่ากับ 171.14 บาท/ครั้ง งานอุดฟันด้วยอมัลกัม 2 ด้าน เท่ากับ 204.04 บาท/ครั้ง งานอุดฟันด้วยวัสดุสีเหมือนฟัน 1 ด้านเท่ากับ 201.92 บาท/ครั้ง งานอุดฟันด้วยวัสดุสีเหมือนฟัน 2 ด้านเท่ากับ 232.79 บาท/ครั้ง ผลจากการศึกษาสามารถนำไปใช้ในการวางแผนการจัดการทรัพยากรและการบริหารงานที่มีประสิทธิภาพของหน่วยงานทันตสาธารณสุข

Abstract: The objective of this study was to analyze itemized unit cost of dental service of Sanmaket Health promoting hospital, Chaing Rai province. Dental services consisted of (1)Extraction; (2) Scaling; (3) Amalgam filling 1 surface; (4) Amalgam filing 2 surfaces ;(5) Resin composite filling 1 surface ;(6) Resin composite filling 2 surfaces. The data in this study was retrospectively collected from October 2016 to November 2017 and collected data from dental JHCIS Program, material disbursement, dental personnel’s remuneration and welfare, dental equipment and building-construction in fiscal year 2017. The collected data were based on labor costs, material costs, and capital costs. To analyze the costs of dental service using activity based costing. The results revealed that unit cost of dental services were lower than the dental service fee of Public Health Service Units, Ministry of Public Health. Unit cost of (1) Extraction was 137.05 Baht/visit; (2) Scaling was 149.93 Baht/visit; (3) Amalgam filling 1 surface was 171.14 Baht/visit; (4) Amalgam filling 2 surfaces was 204.04 Baht/visit; (5) Resin composite filling 1 surface was 201.92 Baht/visit; (6) Resin composite filling 2 surfaces was 232.79 Baht/visit. In addition, the outcomes of this study were utilized for further enhancement of resources and process management in Public Health Dentistry Department.
