Adverse drug reaction monitoring of aminoglycosides in Rajavithi hospital

Abstract: Aminoglycosides are active against a wide range of aerobic gramnegative microorganisms. Due to their potential nephrotoxicity and ototoxicity, they should be used in the treatment of serious, often life threatening, systemic infections. Prospective study of adverse drug reaction (AD) monitoring of aminoglycosides in Rajavithi Hospital was conducted during the periods of June - September 1994 and March October 1995 in patients admitted in Medicine, Orthopedics, Surgery and Urology Wards in order to determine the incidence of the ADRs by intensive hospital monitoring method. Serum aminoglycoside concentrations were also measured in some patients to correlate with their adverse effects. There were 298 patients (72.1% males), age range of 15-85 years old, receiving aminoglycoside therapy either for treatment or prophylaxis. Gentamicin was most commonly prescribed (84.9%) in all wards, with varying dosage regimens. Amikacin and netilmicin were also used (7.1% and 6.4%, respectively) in all wards except the Orthopedics. Streptomycin was prescribed only for patients suffering from tuberculosis. In this study, kanamycin was prescribed only once. Once-daily dosage regimens were also prescribed along with the more frequent, conventional divided doses. The prescriptions were based on the physicians preferences. The results suggested that aminoglycosides seemed to be rationally prescribed. The overall incidence of ADRs occurred in the patients receiving aminoglycoside treatment was 11.7%. Patients treated with netilmicin seemed to experience more adverse reactions (15.8%) than those treated with gentamicin and amikacin (10.3% and 9.5%, respectively). Males and females, and patients in all age groups were equally liable to develop ADRs. Fourty-eight clinical manifestations were reported from 35 patients. Reactions at the site of injection were most commonly found especially in patients receiving gentamicin injections. The incidences of nephrotoxicity and ototoxicity due to aminoglycoside therapy were 4.0% and 2.3%, respectively. Only nephrotoxic reactions, i.e. increase in serum creatinine and BUN, were found in all 5 aminoglycosides studied. Netilmicin and amikacin seemed to be devoid of ototoxicity. Hypersensitivity reactions and nausea were rarely reported. Causality assessment of ADRs by Naranjos algorithm revealed that aminoglycosides may probably be the cause of these reactions. Most of them were considered to be minor which need no specific treatment. Only 5 patients had moderate ADRs which required certain treatments. Serum amikacin and gentamicin concentrations were measured in 16 and 60 patients, respectively. It was found that peak and trough levels were not correlated with the toxicities reported. Therapeutic drug monitoring for aminoglycosides was not shown to be cost-effective as shown in this study. It may not be necessary as a routine service for all patients who received aminoglycoside treatment, if frequent monitoring of serum creatinine and dosage adjustment in the presence of renal impairment are routinely performed in order to avoid the occurrence of both nephrotoxicity and ototoxicity.

Abstract: อะมิโนกลัยโคไซด์ เป็นยาที่มีประสิทธิภาพสูงใน การรักษาโรคติดเชื้อที่เกิดจากเชื้อแบคทีเรียชนิดกรัมลบ แต่เนื่องจากยาในกลุ่มนี้มีความเป็นพิษสูงต่อไตและหู จึงควรใช้เฉพาะในผู้ป่วยที่มีการติดเชื้อที่รุนแรงและอาจ เป็นอันตรายถึงชีวิต ได้ทำการศึกษาติดตามอาการไม่พึงประสงค์จากการ ใช้ยาในกลุ่มอะมิโนกลัยโคไซด์ในผู้ป่วยที่เข้ารับการ รักษาในหอผู้ป่วยอายุรกรรม ศัลยกรรม โรคข้อและกระดูก และทางเดินปัสสาวะของโรงพยาบาลราชวิถีระหว่างเดือน มิถุนายน-กันยายน 2537 และเดือนมีนาคม-ตุลาคม 2538 เพื่อหาอุบัติการณ์ของการเกิดอาการไม่พึงประสงค์จากการ ใช้ยาโดยใช้วิธีการติดตามผู้ป่วยในโรงพยาบาล และได้เก็บ ตัวอย่างเลือดเพื่อวิเคราะห์หาระดับอะมิโนกลัยโคไซด์ ในเลือด เพื่อหาความสัมพันธ์ของระดับยากับการเกิดอาการ ไม่พึงประสงค์ ได้เก็บข้อมูลผู้ป่วย 298 คน (72.1% ของผู้ป่วย เป็นเพศชาย) อายุระหว่าง 15-85 ปี ที่ได้รับยาในกลุ่ม อะมิโนกลัยโคไซด์เพื่อการรักษาหรือป้องกัน พบว่ายา เจนตามัยซินเป็นยาที่มีการใช้บ่อยที่สุดในทุกหอผู้ป่วย (84.9%) ในขนาดของยาที่แตกต่างกัน มีการใช้อะมิเคซิน และเนทิลมัยซินในผู้ป่วย 7.1% และ 6.4% ตามลำดับ ในทุกหอผู้ป่วยที่ศึกษา ยกเว้นหอผู้ป่วยโรคข้อและกระดูก สเตรปโตมัยซินใช้ในผู้ป่วยวัณโรคเท่านั้น กานามัยซิน มีใช้เพียงหนึ่งครั้งในผู้ป่วยเพียงรายเดียวตลอดระยะ เวลาการศึกษา ขนาดยาและช่วงเวลาของการให้ยา แต่ละชนิดขึ้นกับดุลยพินิจของแพทย์ ซึ่งในโรงพยาบาลนี้ มีการใช้ยากลุ่มอะมิโนกลัยโคไซด์ทั้งแบบให้วันละครั้งและ แย่งให้วันละหลายครั้ง อย่างไรก็ตามพบว่าการใช้ยาใน กลุ่มนี้เป็นไปอย่างเหมาะสม ผู้ป่วย 11.7% เกิดอาการไม่พึงประสงค์จากการ ใช้ยาในกลุ่มอะมิโนกลัยโคไซด์ หากแบ่งตามชนิดของยา ที่ใช้จะพบว่า อุบัติการณ์ที่เกิดจากการใช้ยาอะมิเคซิน เจนตามัยซิน และเนทิลมัยซินคือ 9.5%, 10.3% และ 15.8% ตามลำดับ การเกิดอาการไม่พึงประสงค์นี้พบได้ เท่า ๆ กันในทั้งสองเพศและทุกกลุ่มอายุ อาการแสดงที่พบมีทั้งหมด 48 อาการจากผู้ป่วย 35 ราย ปฏิกิริยาที่เกิดบริเวณที่ฉีดยาเป็นอาการที่มี รายงานมากที่สุด โดยเฉพาะในผู้ป่วยที่ได้รับการฉีด เจนตามัยซิน อาการพิษต่อหูพบในผู้ป่วย 2.3% ส่วนอาการ พิษต่อไตพบในผู้ป่วย 4.0% อาการพิษต่อไตอันได้แก่การ เพิ่มของระดับ BUN และครีเอตินินในเลือดเกิดในผู้ป่วย ที่ใช้ยาที่ศึกษาในกลุ่มนี้ทุกตัว แต่ไม่มีรายงานการเกิด พิษต่อหูในผู้ป่วยที่ได้รับเนทิลมัยซินและอะมิเคซิน นอกจาก นี้ยังพบว่ามีรายงานการเกิดอาการแพ้และอาการคลื่นไส้ ในผู้ป่วยไม่มากราย การศึกษาความสัมพันธ์ของสาเหตุการเกิดอาการ ไม่พึงประสงค์จากการใช้ยา โดยใช้ Naranjos algorithm พบว่ายาในกลุ่มอะมิโนกลัยโคซัยด์อาจเป็นสาเหตุของการเกิด อาการเหล่านั้น ด้วยความเชื่อมั่นในระดับ "probable" อาการที่เกิดส่วนใหญ่ไม่ค่อยมีความรุนแรงมีเพียงผู้ป่วย 5 คนเท่านั้น ที่มีอาการรุนแรงปานกลางและต้องการ การรักษา ในการวัดระดับยาอะมิเคซินและเจนตามัยซินในตัวอย่าง เลือดของผู้ป่วยจำนวน 16 และ 60 คน ตามลำดับ พบว่า ระดับยาในเลือดไม่ว่าจะเป็นระดับยาสูงสุดหลังให้ยา (peak) หรือระดับยาต่ำสุดก่อนได้รับยา (trough) ไม่มีความสัมพันธ์ กับอาการพิษที่เกิดขึ้น การวัดระดับยาในเลือดเพื่อประเมินอาการพิษที่เกิด จากยา ไม่น่าจะมีประโยชน์หรือมีประสิทธิผลคุ้มค่ามากนัก ในกรณีของยาในกลุ่มอะมิโนกลัยโคไซด์ และอาจไม่จำเป็นต้อง วดหากมีการวัดระดับครีเอตินินในเลือดบ่อยพอ และปรับขนาด ของยาในกลุ่มอะมิโนกลัยโคไซด์ให้เหมาะสมในผู้ป่วยที่มี การทำงานของไตไม่ปกติ เพื่อหลีกเลี่ยงอาการพิษที่อาจเกิด ต่อไตและหู
