Characterization of the effect of probenecid on hepatic drug metabolism in rats

Abstract: The characteristics of hepatic drug-metabolizing enzyme induction by probenecid, a uricosuric drug, were studied in adult male Wistar rats weighing 140 g to 200 g. Repeated administration of probenecid (600 mg/kg, PO, twice daily for 5 days) could significantly shorten the duration of action of two model drugs, zoxazolamine (60 mg/kg, IP, P-450 IA, IIB/C/E probe) and hexobarbital (120 mg/kg, IP, P-450 IIB/C probe). Determination of the activities of hepatic aminopyrine N-demethylase (P-450 IIB & IIIA probe) and aniline hydroxylase (P-450 IIE probe) revealed that the inductive effect of probenecid was both dose- and time-dependent. During the 5-day period of treatment, maximal induction of both enzymes could be achieved when the drug was given at the dose of 600 mg/kg, PO, twice daily, for 5 days. However, significant induction of aminopyrine N-demethylase could be detected even at 1 day after drugs administration. Aniline hydroxylase was significantly induced only when the drug was given for 5 days in the dose range between 200 and 800 mg/kg. After cessation of probenecid, aniline hydroxylase activity rapidly declined towards normal level while that of aminopyrine N-demethylase was gradually decreased. Induction of both enzymes seemed to be due to quantitative increase in the enzyme content rather than a qualitative change because Kms of both enzymes were essentially the same as those observed in the control animals. Repeated administration of probenecid did not result in the induction of ethoxyresorufin O-deethylase (P-450 IA1 probe) at any time after drug administration. On the contrary, the activity of ethoxycoumarin O-deethylase (nonspecific substrate of P-450 subfamily II) showed timecourse of induction similar to that observed with aminopyrine N-demethylase. Glucuronidation towards l-naphthol and 4-methylumbelliferone, the substrates metabolized by isozymes induced by both the phenobarbital (PB)- and 3-methylcholanthrene (3-MC)-type inducers, was also increased after the animals received the same treatment, though with different characteristics and at different degree. The ultrastructure of hepatocytes from probenecid-treated rats showed a marked proliferation of smooth endoplasmic reticulum, the finding similar to the morphological changes reported after exposure to the PB-type inducers. Overall results suggested that probenecid possessed not only the induction characteristics of phenobarbital but was also capable of inducing the metabolism of a variety of substrates other than that induced by this group of chemicals.

Abstract: ได้ทำการวิจัยลักษณะการเหนี่ยวนำเอ็นซัยม์ที่ เปลี่ยนแปลงยาในตับหนูขาวพันธุ์ Wistar เนื่องจากยา โปรเบเนซิด โดยศึกษาทั้งในสัตว์ทดลองและหลอดทดลอง การศึกษาในสัตว์ทดลองพบว่า การให้โปรเบเนซิด 600 มิลลิกรัม ต่อน้ำหนักตัว 1 กิโลกรัม โดยทางปากวันละ 2 ครั้ง เป็น เวลา 5 วัน สามารถลดระยะเวลาการออกฤทธิ์ของยาคลาย กล้ามเนื้อ zoxazolamine (ขนาด 60 มิลลิกรัมต่อน้ำหนักตัว 1 กิโลกรัม) และระยะเวลาการนอนหลับจากการให้ยานอนหลับ hexobarbital (ขนาด 120 มิลลิกรัม ต่อน้ำหนักตัว 1 กิโลกรัม โดยการฉีดเข้าช่องท้องหนู) การศึกษาในหลอดทดลองพบว่า การให้ยาโปรเบเนซิด ขนาด 600 มิลลิกรัมต่อน้ำหนักตัว 1 กิโลกรัม โดยทางปาก วันละ 2 ครั้ง เป็นเวลา 5 วัน สามารถเหนี่ยวนำเอ็นซัยม์ aminopyrine N-demethylase และเอ็นซัยม์ aniline hydroxylase โดยความสามารถในการเหนี่ยวนำขึ้นกับระยะ เวลาและขนาดของยาโปรเบเนซิดที่ให้แก่สัตว์ทดลอง พบว่าเอ็นซัยม์ทั้งสองสามารถเหนี่ยวนำได้สูงสุดโดย ยาโปรเบเนซิดขนาด 600 มิลลิกรัม ต่อน้ำหนักตัว 1 กิโลกรัม และให้ยาเป็นระยะเวลา 5 วันติดต่อกัน แต่การเหนี่ยวนำ เอ็นซัยม์ aminopyrine N-demethylase สามารถเกิดขึ้นได้ แม้ใช้ยาโปรเบเนซิดเพียงขนาด 200 มิลลิกรัม ต่อน้ำหนักตัว 1 กิโลกรัม ถ้าใช้ขนาดยาสูงจะเห็นการเหนี่ยวนำชัดเจน หลังให้ยาเพียง 1 วัน ส่วนเอ็นซัยม์ aniline hydroxylase สามารถเหนี่ยวนำได้โดยยาโปรเบเนซิดในขนาด 200-800 มิลลิกรัม ต่อ น้ำหนักตัว 1 กิโลกรัม เมื่อให้ยาติดต่อกัน เป็นเวลา 5 วันเท่านั้น หลังจากหยุดให้ยาโปรเบเนซิดระดับ การทำงานของเอ็นซัยม์ aniline hydroxylase ลดลง สู่ระดับปกติอย่างรวดเร็ว ในขณะที่ระดับการทำงานของ เอ็นซัยม์ aminopyrine N-demethylase ลดลงอย่างช้า ๆ จากการศึกษาจลนศาสตร์ของเอ็นซัยม์ทำให้ทราบว่าเอ็นซัยม์ ที่ทำหน้าที่ในปฏิกิริยา aminopyrine N-demethylation และ aniline hydroxylation ในหนูปกติและหนูซึ่งได้รับ ยาโปรเบเนซิดเป็นเอ็นซัยม์ตัวเดียวกัน นอกจากนี้การให้ยาโปรเบเนซิดในขนาดดังกล่าว ข้างต้นยังสามารถเหนี่ยวนำเอ็นซัยม์ ethoxycoumarin O-deethylase (IIA & IIB probe) ซึ่งลักษณะการเหนี่ยว นำคล้ายกับการเหนี่ยวนำเอ็นซัยม์ aminopyrine N-demethylase แต่ไม่สามารถเหนี่ยวเอ็นซัยม์ ethoxyresorufin O-deethylase (IA1 probe) นอกจากเอ็นซัยม์ซึ่งเป็นกลุ่ม cytochrome P-450 แล้วยาโปรเบเนซิดยังสามารถเหนี่ยวนำเอ็นซัยม์ที่ทำหน้าที่ ในปฏิกิริยา glucuronidation ของ 1-naph-thol และ 4-methylumbelliferone ได้ดีอีกด้วยเมื่อให้ยาในขนาด และวิธีเดียวกัน จากการศึกษาการเปลี่ยนแปลงของเซลล์ตับโดยจุลทรรศน์ อิเล็คตรอนพบว่า ในสัตว์ทดลองที่ได้รับยาโปรเบเนซิดเซลล์ ตับมี smooth endoplasmic reticulum เพิ่มขึ้นอย่างเห็น ได้ชัด ซึ่งลักษณะของการเปลี่ยนแปลงดังกล่าวคล้ายกับการเปลี่ยน แปลงของเซลล์ตับในสัตว์ทดลองที่ได้รับ phenobarbital จากการศึกษาทั้งหมดสามารถสนับสนุนว่า การเหนี่ยวนำ เอ็นซัยม์โดยยาโปรเบเนซิดมีลักษณะคล้ายกันกับการเหนี่ยว นำเอ็นซัยม์โดย phenobaribital แต่มีความสามารถในการ เหนี่ยวนำเอ็นซัยม์ซึ่งทำลายยาตัวอื่น ๆ บางอย่างได้แตกต่างกัน
