Abstract: การศึกษาครั้งนี้มีวัตถุประสงค์ เพื่อศึกษาต้นทุนทั้งหมด ต้นทุนต่อหน่วย อัตราส่วนของต้นทุนค่าแรง ต้นทุนค่าวัสดุ และต้นทุนค่าลงทุน รายได้และการคืนทุนของผู้ป่วยนอก หน่วย ไตเทียม โรงพยาบาลเชียงรายประชานุเคราะห์ จังหวัดเชียงราย ปีงบประมาณ 2557 โดยทำการศึกษาแบบย้อนหลัง ในทัศนะของผู้ให้บริการ เครื่องมือที่ใช้ในการศึกษาเป็นแบบบันทึกต้นทุนค่าแรง ค่าวัสดุ ค่าลงทุน โดยจัดแบ่งหน่วยต้นทุนในส่วนที่เกี่ยวข้องในโรงพยาบาลออกเป็น 3 กลุ่มคือ หน่วยต้นทุนที่ไม่ก่อให้เกิดรายได้ (NRPCC) หน่วยต้นทุนที่ก่อให้เกิดรายได้ (RPCC) และหน่วยงานที่บริการผู้ป่วยโดยตรง (PS) ต้นทุนแบ่งออกเป็นต้นทุนทางตรงและต้นทุนทางอ้อม ต้นทุนทางอ้อมจัดสรรโดยวิธีกระจายต้นทุนแบบขั้นบันได ต้นทุนต่อหน่วยวิเคราะห์จากต้นทุนรวมหารด้วยจำนวนครั้งของผู้ใช้บริการผู้ป่วยนอกงานไตเทียม อัตราคืนทุนวิเคราะห์จากรายรับหารด้วยต้นทุนทั้งหมด ผลการศึกษาพบว่า แผนกบริการผู้ป่วยนอกงานไตเทียมมีต้นทุนที่เกิดขึ้นทั้งสิ้น 73,667,236.90 บาท ต้นทุนเฉลี่ยต่อครั้งของการให้บริการของแผนกบริการผู้ป่วยนอกงานไตเทียม ปีงบประมาณ 2557 เป็นมูลค่า 1,468.50 บาท มีผู้ป่วยมารับบริการฟอกเลือดด้วยเครื่องฟอก ไตเทียม 50,164 ครั้ง โดยมีต้นทุน Routine service cost (RSC) เท่ากับ 1,390.29 บาท/ครั้ง และมีต้นทุน Medical care cost (MCC) เท่ากับ 78.21 บาท/ครั้ง และคิดเป็นต้นทุนทางตรง 1,383.90 บาท และต้นทุนทางอ้อมจากหน่วยงานที่สนับสนุน 84.65 บาท โดยมีต้นทุนค่าแรง 145.50 บาท ต้นทุนค่าวัสดุ 1,171.90 บาท ต้นทุนค่าลงทุน 66.50 บาท โดยต้นทุนค่าแรงพยาบาลต่อครั้งคือ 139.63 บาท ต้นทุนค่าเสื่อมของเครื่องฟอกไตต่อครั้ง คือ 55.82 บาท ต้นทุนบริการประจำ (Routine Service Cost) ต่อหน่วย คือ 1,414.42 บาทต่อครั้ง โดยส่วนใหญ่เป็นต้นทุนของค่าวัสดุ รองลงมาเป็นต้นทุนค่าแรง และต่ำสุดเป็นต้นทุนของค่าลงทุน คิดเป็นร้อยละ 10.8 : 80.8 : 8.5 ตามลำดับ หรือเทียบเป็นสัดส่วนคือ ต้นทุนค่าแรง: ค่าวัสดุ: ค่าลงทุน เท่ากับ 1.3 : 9.5 : 1.0 ตามลำดับ การคืนทุนของแผนกบริการผู้ป่วยนอกงานไตเทียมโรงพยาบาลเชียงรายประชานุเคราะห์ ปี 2557 ในมิติของรายได้ที่เรียกเก็บทั้งหมดเท่ากับร้อยละ 102.20 และ ในส่วนการคืนทุนของรายได้สุทธิเท่ากับร้อยละ 96.7 การศึกษาครั้งนี้สรุปได้ว่า ต้นทุนค่าวัสดุมีอิทธิพลสูงสุดต่อต้นทุนรวมทั้งหมดของแผนกบริการผู้ป่วยนอกงานไตเทียม ซึ่งต้นทุนต่อหน่วยของแผนกบริการผู้ป่วยนอกงานไตเทียมมีค่าต่ำกว่าต้นทุนที่เรียกเก็บจากโครงการหลักประกันสุขภาพถ้วนหน้า แต่รายรับของงานไตเทียม ไม่สามารถรองรับต้นทุนทั้งหมดของงานไตเทียมได้ ดังนั้นข้อมูลเหล่านี้ถือเป็นข้อมูลพื้นฐาน การบริหารทรัพยากรและดำเนินการควบคุมต้นทุนให้มีประสิทธิภาพต่อไป

Abstract: This research was aimed to identify the total cost, unit cost, labor cost, material cost, investment cost and income and cost recovery of Hemodialysis Center for out-patients department of Chiang Rai Prachanukroh Hospital, Chiang Rai Province, in the fiscal-year 2014. The research was conducted in provider perspective. The recording forms of labor cost, material cost, and capital cost were used for collecting data. Cost analysis of this study was divided into 3 groups including Non-Revenue Producing Cost Center (NRPCC), Revenue Producing Cost Center (RPCC) and Patient Service (PS). The total cost was consisted of direct and indirect costs. The indirect cost was allocated by using the step-down method. The unit cost was analyzed from total cost divided by the number of out-patients visits in the fiscal-year2014 of out-patients of Hemodialysis service. The cost recovery rate was analyzed from the revenue divided by total cost. The result of this study, the total cost of Hemodialysis center was 73,667,236.90 bahts. There were patients getting Hemodialysis service for 50,164 visits. The average cost per visits of Hemodialysis center for out-patients in the fiscal-year 2014 was 1,468.50 Baht. The Routine Service Cost (RSC) was 1,390.29 Baht/ visits and the Medical Care Cost (MCC) was 78.21 Baht/ visits. The direct cost was 1,383.90 while the indirect cost from a supporting organization was 84.64 Baht. Other costs were as follows; labor cost: 145.50 Baht, material cost: 1,171.90 Baht, investment cost: 66.50 Baht and nursing service cost per time: 139.63 Baht. Hemodialysis machine depreciation cost per visits was 55.82 Baht. The highest amount was the material cost, followed by labor cost and investment cost at 10.76%: 80.74%: 8.50%, or in the ratio of 1.3 : 9.5 : 1.0 respectively. The cost recovery of Hemodialysis Center for out-patients in Chiang Rai Prachanukroh Hospital in 2014 in terms of Accrual Cost Recovery: ACR was 103.4%. The cost recovery in terms of net income was 96.7 %. Considering operation cost recovery, collected revenue recovery and net income were 537.0 and 502.4 %. The cost recovery of material cost of the collected revenue and net income was 128.1 and 119.8 % Finally, the material cost had the highest influence on total cost of Hemodialysis Center for out-patients. The cost per unit of Hemodialysis Center for out-patients was lower than the cost collected from the Comprehensive Insurance Scheme. So this information is the Basic information to manage resources and control total cost efficiency.
