Abstract:
Background : Melasma is a common pigmentary disorder often seen in patients with skin phototypes IIIVI, especially in pregnant women and the people more exposed to the sun. Clinically can help diagnosis, but it is difficult to treat. Rhubarb liquid extract also called Rheum emodi, has an antioxidant effect, antityrosinase and skin lightening effect. Thus, it would be a new topical herb for melasma treatment. However, no clinical study show topical herbal rhubarb extract liquid for the treatment of melasma in volunteer.
Objective: A randomized, split face comparative trial was performed to determine an efficacy of topical herbal Rhubarb liquid for treatment of melasma in volunteer.
Material and methods: Thirty patients (men and women, aged 20-60 years old) with melasma were enrolled to this randomized, split face comparative study. The patients were enrolled and randomly assigned 5% Rhubarb extract liquid on one side and placebo on the other side twice daily for 8 weeks with follow up every 4 weeks. Mexameter measurement were perfomed along with photography by VISIA Analysis at baseline, 4th week, 8th week and 12th week. Adverse events and satisfaction score were observed and recorded throughout the study. This is possible due to fact that all volunteers suggested to use sunscreen until this study finished.
Result: Twenty-nine patients have completed the study. Seventy-four percent of patients does not have family history of melasma. Mean melanin index of the sides between Rhubarb extract liquid which is from 230.41±58.56 to 201.00±55.80 and placebo liquid from 231.52±59.29 to 221.17±62.80, P-value of Rhubarb extract liquid group is 0.46 which demonstrated statistically significant reduced from the baseline to 12th week correspondingly. The researcher did the the following at significance level of p-value less than 0.05, it is mean 5% rhubarb extract liquid for treatment melasma is significant.
Most volunteers rated rhubarb extract liquid as much improvement for satisfactory without serious side effects. The p-value compared between Rhubarb and placebo liquid on 12th week less than 0.001. The researcher did following at significane level of P-value less than 0.05.
After being treated the side effects were found in two (6.9%) patients with rhubarb extract liquid user and 3(10.3%) patients with placebo user, but the reason is volunteer forgot use sun-scream.
Conclution: Through a self-controlled study of the left and right face, we can determine whether rhubarb extract liquid active ingredients can be applied to improve and prevent facial hyperpigmentation. Following twelve weeks of clinical data and studies, rhubarb extract liquid could have significant better than placebo from 8th week onwards. Rhubarb extract liquid is a safe treatment options for patients with melasma.
