Relationship between Liver Function and Hormonal Steroid Contraception in Hepatitis B Virus Carriers

Abstract: This study aimed to find the effect of using low-dose combined oral contraceptives compared with intrauterine contraceptive devices on the changes in liver functions in chronic hepatitis B carriers who had normal liver functions. Out of 164 cases for serological study, 27 cases (16.46%) had negative results for HBsAg. The serological study of 137 cases of chronic hepatitis B carriers indicated that all of them had anti-HBc 54 cases (32.93%) with Hbe Ag positive, 68 cases (41.46%) with anti-HBe positive, and only 2 cases with antiHBs positive. Blood testing for liver function performed before using contraceptives showed that 15 of 137 cases (10.95%) had SGPT > 100 u/dl. Out of 122 cases being HBsAg positive and having normal liver functions, 10 cases were excluded - 4 cases discontinued, and 6 cases were lost during follow up; hence, there were 59 IUD users and 53 COC users who participated in this study. The liver function tests performed 3 months after using contraceptive showed that bilirubin 1 in COC users were higher than in IUD users, the statistical difference of p-value was 0.0315; however, after using contraceptives for 6 months, the results of liver function tests showed no diference. Thus, this study confirmed that low-dose combined oral contraceptives do not affect liver function in hepatitis B carriers who have normal liver functions or show no clinical symtoms
