Abstract:
Interferon (IFN) has been shown to induce hematologic responses in chronic myelogenous leukemia (CML) and complete suppression of Ph' positive cells has been reported. Therapeutic effectiveness, in terms of eradication of Ph' positive cells and abnormal bcr-abl, of interferon alpha-2a (IFNa) and hydroxyurea (HU) was examined and compared with bone marrow transplantation (BMT). The breakpoint distribution within M-bcr and the relationship between the site of breakpoint and clinical and hematological features are reported. Repeated treatment with combined HU+IFNa for up to one year failed to achieve cytogenetic remission or complete eradication of bcr-abl rearranged cells in all patients. Despite this, the majority of the patients showed a reduction in the number of cytogenetically abnormal cells in the blood and bone marrow which was confirmed by a decrease in intensity of the bcr rearranged band. In patients receiving BMT, disappearance of Ph' positive cells and M-bcr rearrangement was observed within 2 months post BMT in 6 out of 8 and 5 out of 6 patients, respectively, for up to 2 years observation. Of all 15 patients, 5 had a break within the 5' region of M-bcr and 10 within the 3' region. There was no apparent correlation between M-bcr breakpoint location and clinical characteristics or hematological features at diagnosis
