The association between periodontal disease and acute coronary syndrome
Abstract:
Background: The association between periodontal diseases and coronary heart
disease (CHD) has been suggested by several studies. The aim of this study was to
investigate the association between periodontal diseases and acute coronary syndrome
(ACS) in a matched case-control design.
Methods: Seventy ACS patients admitted to the Buddhachinaraj Hospital,
Phitsanulok, Thailand, were matched (age and gender) with 70 control subjects with
no history of CHD from the communities. General information and medical history of
both groups were obtained from questionnaire and interview. All participants
underwent complete oral examination to determine gingival index, pocket depth,
gingival recession and clinical attachment loss (CAL). The association between
periodontal diseases and ACS was analyzed using conditional multiple logistic
regression analysis.
Results: Severity of periodontitis was significantly associated with ACS after
adjustment for age, gender, smoking, alcohol and exercise. The adjusted odds ratios
were 6.7 [95% confidence interval (CI) 1.4 to 33.0] for moderate periodontitis, and 7.6
(95% CI 1.8 to 32.3) for severe periodontitis, respectively, when compared to mild
periodontitis. The presence of gingivitis was not associated with ACS in this study.
Conclusions: Periodontal disease characterized by periodontitis, but not gingivitis,
was associated with acute coronary syndrome
