The effects of nursing therapeutics on preventive management of preterm labor readmission and gestational age at birth in pregnant women with preterm labor
Abstract:
This study was a quasi-experimental design aimed at examining the effect of
nursing therapeutics on the preventive management of preterm labor, readmission and
gestational age at birth in pregnant women with preterm labor. The participants
consisted of 45 pregnant women with preterm labor, who were observed in delivery
room at Rajavithi Hospital. They were divided into 2 groups. The 22 pregnant women
in the control group received usual nursing care and the 23 pregnant women in the
experimental group received the usual nursing care plus nursing therapeutics based on
the Transition theory of Meleis and colleagues. The nursing therapeutics emphasized
the management of personal conditions and community conditions which were
composed of the problems and risk factors assessment related to preterm labor,
instructions for preterm labor, a handbook about preterm labor, routine monitoring
symptoms, family education, coordination with obstetrician, home visit telephone and
follow-up at the antenatal clinic. Data collection was gathered by interviews
concerning on demographic and preterm labor risk factors and by using questionnaires
regarding the Preventive Management of Preterm labor. Analysis was performed by
descriptive statistics, Chi-square, t-test, ANCOVA and Fishers exact probability test.
The findings revealed that the experimental group had a score of preventive
management of preterm labor significantly higher than before implementation
(p<.001). The score of preventive management of preterm labor and gestational age at
birth after implementation in the experimental group were significantly higher than the
score in the control group (p<.001, p<.05, respectively). However, the readmission
rate, in the both groups were not statistically significant difference (p>.05).
The results of the study suggested that nurses in the delivery room should
provide nursing therapeutics by emphasizing the management of personal conditions
and community conditions in order to provide pregnant women with preterm labor
counseling and prevent a reoccurrence of preterm labor and extend their pregnancy
until delivery.
