A study on the association of metalworking fluids mist exposure on respiratory function in automotive part workers

การศึกษาความสัมพันธ์จากการสัมผัสละอองน้ำมันของงานโลหะที่มีต่อระบบทางเดินหายใจในคนงานผลิตชิ้นส่วนยานยนต์

Abstract: The research study aimed at studying the association between metalworking fluids mist exposure and respiratory function in automotive part workers. The pulmonary function test were performed in order to find the differences in pre-shift functions between the exposed and unexposed groups. Function test also aimed to find the difference between pre-shift and post-shift functions and predicted values as well as the association between metalworking fluids mist concentrations and cross-shift changes in FVC and FEV1 and respiratory symptoms. The participants in this study were 97 males working in a automotive parts factory located in the Amata Nakorn Industry Estate, Chonburi Province. They were divided into 2 groups, consisting of 56 exposed metalworking fluids mist worker and 41 unexposed. Metalworking fluids mist concentration was measured on the first working days of workweek. The interview questionnaires were applied to collect the respiratory symptoms. The statistical analyses used for this study were Paired t-test, t-test, Spearman Correlation, Pearson Correlation and Chi-square test. The findings revealed that the exposed group had a greater potential for abnormal pulmonary function. There was a significant association between metalworking fluids mist concentration in working areas and cross-shift changes in FVC (r = 0.464, p<0.001) and FEV1 (r = 0.557, p<0.001) and there was significant association between metalworking fluids mist concentration and chest tightness (p=0.01). It could be concluded that the exposed group had potential abnormal pulmonary functions, called restrictive defect. The cross-shift changes in FVC and FEV1 could be the good indices for early detection of acute pulmonary functions because they corresponded with concentrations of the metalworking fluids mist.

Abstract: การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อศึกษาความสัมพันธ์จากการสัมผัสละอองน้ำมันของงานโลหะที่มีต่อ ระบบทางเดินหายใจในพนักงานผลิตชิ้นส่วนยานยนต์ โดยการทดสอบความแตกต่างของสมรรถภาพปอดก่อนเริ่ม งานของพนักงานกลุ่มสัมผัสกับไม่สัมผัสละอองน้ำมันจากงานโลหะ เปรียบเทียบสมรรถภาพปอดก่อนเริ่มงาน และหลังเลิกงานกับค่าพยากรณ์ ศึกษาความสัมพันธ์ระหว่างปริมาณความเข้มข้นของละอองน้ำมันของงานโลหะ กับเปอร์เซ็นต์การเปลี่ยนแปลงระหว่างกะของค่า FVC และ FEV1 และศึกษาความสัมพันธ์ระหว่างปริมาณความ เข้มข้นของละอองน้ำมันของงานโลหะกับการเกิดอาการของโรคระบบทางเดินหายใจ กลุ่มตัวอย่างในการศึกษาครั้งนี้ได้แก่พนักงานเพศชาย ซึ่งทำงานโรงงานผลิตชิ้นส่วนยานยนต์ ที่ตั้งอยู่ใน นิคมอุตสาหกรรมอมตะนคร จังหวัดชลบุรี จำนวน 97 คน แบ่งเป็นพนักงานกลุ่มสัมผัสละอองน้ำมัน 56 คน พนักงานกลุ่มไม่สัมผัสละอองน้ำมัน 41 คน โดยทำการเก็บข้อมูลสมรรถภาพปอดในการทำงานวันแรกของ สัปดาห์ ทำการตรวจวัดปริมาณความเข้มข้นของละอองน้ำมันในสิ่งแวดล้อมการทำงาน และสัมภาษณ์อาการของ โรคระบบทางเดินหายใจโดยการใช้แบบสอบถาม และสถิติที่ใช้ในการวิเคราะห์ได้แก่ Paired t-test, t-test, Spearman Correlation test, Pearson Correlation test and chi-squares test ผลการศึกษา พบว่า พนักงานกลุ่มสัมผัสละอองน้ำมันมีแนวโน้มของสมรรถภาพปอดลดลงมากกว่า พนักงานกลุ่มไม่สัมผัสละอองน้ำมัน ปริมาณความเข้มข้นของละอองน้ำมันในพื้นที่ทำงานมีความสัมพันธ์กับ เปอร์เซ็นต์การเปลี่ยนแปลงของค่า FVC (r = 0.464, p<0.001) และ FEV1 (r = 0.557, p<0.001) และ ปริมาณความเข้มข้นของละอองน้ำมันจากงานโลหะมีความสัมพันธ์กับอาการแน่นหน้าอก อย่างมีนัยสำคัญทางสถิติ (p=0.01) และจากผลการศึกษา พบว่า พนักงานกลุ่มสัมผัสละอองน้ำมันมีแนวโน้มของสมรรถภาพปอดผิดปกติ แบบการขยายตัวของปอดจำกัด (Restrictive defect) และจากผลการศึกษายังแสดงให้เห็นว่าการเปลี่ยนแปลงของ ค่า FVC และ FEV1 ระหว่างกะสามารถใช้เป็นตัวชี้วัดการผิดปกติของสมรรถภาพปอดเบื้องต้นในพนักงานที่ สัมผัสละอองน้ำมันของงานโลหะ
