Factors associated with blood pressure control in essential hypertensive patients at Suak primary care unit, Nan province

Abstract: There is a relationship between blood pressure control and risk factors in hypertensive patients. This study explores this relationship with the view to providing helpful indicators for better patient care. A cross-sectional study was designed to determine risk factors of essential hypertensive patients with blood pressure control at Suak Primary Care Unit in Nan Province, who received hypertensive treatment there from 1 February to 30 April 2007. The total number of subjects was 310. Chi-square statistics were used in univariate and multivariate analysis by logistic regression with 95% confidence interval. There were 115 men (37.1%) and 195 women (62.9%) with a mean age of 61.89 years. The factors such as having adequate income and savings (ORadj = 2.51, 95%CI = 1.14-5.56, p = 0.023) and having no one to remind them to meet doctors or nurses (ORadj = 1.95, 95%CI = 1.14-3.32, p = 0.015) were significantly associated with blood pressure control in this study. The finding of this study shows the factors associated with hypertension in the Suak Primary Care Unit and provides useful knowledge for prevention and control of essential hypertension.

Abstract: การศึกษาชนิด CROSS-SECTIONAL นี้ มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีผลต่อ การ ควบคุมระดับความดันโลหิตของผู้ป่วยความดันโลหิตสูงชนิดไม่ทราบสาเหตุ ในหน่วยบริการปฐม ภูมิตำบลสวก จังหวัดน่าน ระหว่างวันที่ 1 กุมภาพันธ์ ถึง 30 เมษายน 2550 กลุ่มตัวอย่างที่ศึกษาคือ ผู้ป่วยความดันโลหิตสูงชนิดไม่ทราบสาเหตุ ที่มารับการรักษาไม่น้อยกว่า 6 เดือน จำนวน 310 คน วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา การทดสอบค่าไคว์-สแคว์ (Chi-square test) ในการ วิเคราะห์ตัวแปรเดี่ยวและการวิเคราะห์ตัวแปรเชิงซ้อน โดยการใช้ Multiple logistic regression เพื่อทดสอบความสัมพันธ์ โดยดูนัยสำคัญทางสถิติ และใช้ค่าความเชื่อมั่น 95 เปอร์เซ็นต์ ผลการศึกษาพบว่ากลุ่มตัวอย่างเป็นเพศชายร้อยละ 37.1 และเพศหญิงร้อยละ 62.9 อายุ เฉลี่ย 61.89 ปี เมื่อวิเคราะห์ด้วยตัวแปรเชิงซ้อน (Multiple logistic regression) โดยควบคุม อิทธิพลของปัจจัยตัวอื่นๆพบว่า ปัจจัยที่มีความสัมพันธ์กับการควบคุมระดับความดันโลหิตของ ผู้ป่วยความดันโลหิตสูงชนิดไม่ทราบสาเหตุอย่างมีนัยสำคัญทางสถิติ ได้แก่ ความพอเพียงของ รายได้ (ORadj = 2.51, 95%CI = 1.14-5.56, p = 0.023) และ การไม่มีคนคอยเตือนให้ไปพบ แพทย์/พยาบาล (ORadj = 1.95, 95%CI = 1.14-3.32, p = 0.015) จากผลการศึกษาในครั้งนี้แสดงให้เห็นถึงปัจจัยที่มีผลต่อการควบคุมระดับความดันโลหิตของ ผู้ป่วยความดันโลหิตสูงชนิดไม่ทราบสาเหตุในหน่วยบริการปฐมภูมิตำบลสวก จังหวัดน่าน จึงควร นำผลที่ได้ไปใช้ในการป้องกันและควบคุมโรคความดันโลหิตสูงชนิดไม่ทราบสาเหตุต่อไป
