Gift exchange and "SI" in pharmaceutical representative and physician relationships

การแลกเปลี่ยนและความ "ซี้" ในความสัมพันธ์ระหว่างผู้แทนยาและแพทย์

Abstract: This study aims to explore the relationships between physicians and pharmaceutical sales representatives, and the processes to build that relationships which bring about understanding toward influence of pharmaceutical representative to physician’s prescription. This study is qualitative study which main data was collected from indepth interview, observation, and document review. The key informants were 9 physicians, 11 pharmaceutical representatives, and 5 pharmacy students in Khon Kaen province. The data was collected during June to July 2003 and October to December 2003. The finding indicated that even practicing of medicine was based on knowledge, rationality concept, but the representative influenced on physician’s prescription through employing personal relationship. This resulted medicine rationality was partly true. There were several patterns of this relationship but the crucial one for success in their job was “Si”. “Si” was the relationship between physician and pharmaceutical representative that look like generous or close relationship. However, the representative manipulated “Si” in order to gain their calculated interest. Gift exchange was used to formulate “Si”. This gift exchange had to be performed under the norm which is as implicit rule that both the physician and the representative were all knew. The norm was the rule that indicated what should or should not be done. The representative who did not conform to the norm “Si” could not be built. The fundamental logic of this norm is offense. The representative, therefore, had to obliquely perform in order to make the physician not perceived that they were offended. To show interest explicitly such as to eagerly urge physician to use their drug, to explicitly show strict conditions of repay back to representative when receive something; was to offend. Successful pharmaceutical representative understand and comply this norm as a part of their everyday life.

Abstract: การวิจัยนี้มุ่งหวังจะศึกษาถึงความสัมพันธ์ระหว่างผู้แทนยาและแพทย์ ตลอดจนกระบวนการเกิด ความสัมพันธ์ดังกล่าว ทั้งนี้เพื่อนำไปสู่ความเข้าใจต่ออิทธิพลที่ผู้แทนยามีต่อการตัดสินใจใช้ยาของแพทย์ การศึกษานี้เป็นการวิจัยเชิงคุณภาพที่ข้อมูลหลักได้จากการสัมภาษณ์เจาะลึก การสังเกตและจากเอกสาร ผู้ให้ข้อมูลหลักเป็นแพทย์ 9 คน ผู้แทนยา 11 คนและนักศึกษาเภสัชศาสตร์ 5 คนในจังหวัดขอนแก่น ระหว่าง เดือนมิถุนายนถึงกรกฏาคม พ.ศ. 2547 และระหว่างเดือนตุลาคมถึงธันวาคม พ.ศ. 2547 รวมระยะเวลา 4 เดือน ผลการศึกษาพบว่าถึงแม้การแพทย์ประกอบวิชาชีพโดยอาศัยหลักการเหตุผล แต่อิทธิพลจากผู้แทนยาที่ อาศัยรูปแบบของความสัมพันธ์ส่วนตัวเข้าไปมีอิทธิพลต่อการใช้ยาของแพทย์ ทำให้หลักการของเหตุผลเป็นจริง เพียงบางส่วน ความสัมพันธ์ระหว่างแพทย์และผู้แทนยามีหลายรูปแบบ แต่รูปแบบที่สำคัญที่ผู้แทนยาใช้เพื่อ ผลประโยชน์ต่อการทำงานของพวกเขาคือความ “ซี้” ซึ่งเป็นความสัมพันธ์ที่ดูจากภายนอกเหมือนความสัมพันธ์ แบบใจดี แบบใกล้ชิดสนิทสนม แต่เบื้องหลังของความ “ซี้” คือผู้แทนยาจงใจสร้างมันขึ้น ภายใต้ผลประโยชน์ที่ได้ คำนวณไว้ล่วงหน้าแล้ว กระบวนการสร้างความ “ซี้” นี้อาศัยการแลกเปลี่ยนเป็นหลัก การแลกเปลี่ยนนี้เป็นตัวสร้างสาน ความสัมพันธ์ให้เกิดขึ้น โดยต้องปฏิบัติภายใต้บรรทัดฐาน ซึ่งเป็นเสมือนกติกาที่ไม่มีการกำหนดไว้เป็นลายลักษณ์ อักษร แต่ทั้งแพทย์และผู้แทนยาต่างทราบดีว่าอะไรยอมรับได้อะไรไม่ควรปฏิบัติ การฝ่าฝืนกับบรรทัดฐานนี้จะทำ ให้ความ “ซี้” ไม่เกิดขึ้น ตรรกะที่เป็นพื้นฐานสำคัญต่อบรรทัดฐานนี้คือ การคุกคาม ผู้แทนยาจึงต้องหลีกเลี่ยงการแสดงออกถึง การคุกคามต่อแพทย์ การแสดงออกถึงผลประโยชน์อย่างโจ่งแจ้ง เช่น การแสดงออกถึงความต้องให้แพทย์ใช้ยา ของตนเองอย่างโจ่งแจ้ง หรือการกำหนดเงื่อนไขการตอบแทนของแพทย์เมื่อผู้แทนยาให้บางสิ่งไป เป็นการ แสดงออกถึงการคุกคามต่อแพทย์ ผู้แทนยาที่ประสบความสำเร็จจะเข้าใจและปฏิบัติตามบรรทัดฐานนี้เสมือนเป็น ส่วนหนึ่งของชีวิตประจำวัน
