The effectiveness of individual counselling to promote preventive behaviors against complications among post coronary artery bypass graft patients attending Pramongkutklao Hospital
Abstract:
This one-group,pretest – posttest research aimed to evaluate the effectiveness of a health
promotion program to promote preventive behaviors against post operation complications
among post coronary artery bypass graft patients at Pramongkutklao Hospital, Bangkok.
Protection Motivation Theory (PMT) and individual counselling technique were applied to
design a health promotion program. Thirty patients voluntarily participated in this program.
During 16 weeks of intervention, four health promotion activities were successively organized
to promote desirable health behaviors. Data were collected using structured interview, before
and after the intervention, and one month after completion of the intervention. Descriptive
statistics and paired t-test were used for data analysis.
Results from this study showed that the program had effectively enhanced desirable
health behaviors to prevent post operation complications among patients participating in the
health promotion program. Their levels of knowledge, perceived susceptibility, and perceived
severity of post coronary artery bypass graft; perceived self efficacy; and response efficacy,
significantly increased (p < 0.001). Consequently, their health behaviors -eating and exercisealso
significantly improved. These significant improvements remained even during the follow
up period. The average body mass index (BMI) was significantly reduced during the follow up
period (p<0.05), but not at the end of intervention. The average value of blood cholesterol of the
patients reduced, but this reduction was not statistically significant.
Clearly, the application of PMT together with individual counselling could effectively
promote desirable health behaviors among post coronary bypass graft patients. However, BMI and
cholesterol level could not be significantly reduced at the end of the intervention. This might be due to
time limitations and physical conditions of the patients. It is, therefore, recommended that duration of a
program intervention should be extended and other activities such as taking medication and stress
management should be included so that substantial changes can be observed.
