Effects of supportive-educative nursing system on dependent care behaviors and the occurrence of complications among children with long-term tunneled catheler
Abstract:
The purpose of this quasi-experimental research was to find out the effect of a
supportive-educative nursing system on dependent care behaviors and the occurrence
of complications among children with long-term tunneled catheter. The conceptual
framework of this study is based on Orem’s Nursing Theory. Total samples were 30
caregivers of children with long-term tunneled catheter by using purposive sampling at
Ramathibodi Hospital from May 2003 to May 2004. These caregivers were
responsible for changing the catheter exit site dressing when the children were at
home. They were divided equally into a control group and an experimental group.
They were primary caregivers, had no prior experience in catheter care, were willing
to participate in the study and signed an informed consent, and were able to read and
write Thai. The patients were children of varying age, who were having their first
catheter insertion. Both groups obtained conventional nursing care. The experimental
group was given extra supportive-educative nursing system and a handbook. Three
questionnaires on demographic data, caregiving behaviors, and complication
occurrence were applied to collect data at two weeks post discharge. All data were
analyzed by SPSS/FW program.
The results revealed that the score of caregiving behaviors at two weeks post
discharge in the experimental group was significantly higher than in the control group
(p< .001). The occurrence of complications at two weeks was an early sign of exit site
infection in children whose caregivers were in the control group was significantly
higher than in the experimental group (p< .007). No occurrence of partial occlusion
and catheter displacement occurred in either groups.
The supportive-educative nursing system for encouraging catheter caregiving
behavior is effective and tends to reduce catheter related complications. Therefore, its
use is recommended.
