Cost-effectiveness analysis of coronary artery bypass graft (CABG) : a comparison between on-pump CABG versus off-pump CABG
Abstract:
The purpose of this research is to compare the cost effectiveness of two
Coronary Artery Bypass Graft (CABG) surgical procedures: the On Pump CABG
procedure and Off Pump CABG procedure (OPCAB). The data collection was
performed in one Bangkok private hospital over 2 years, between March, 2001 and
March, 2003. The methodology is both quantitative and qualitative.
The sample study group comprised patients with similar pre-operative
histories and was divided into high risk and low risk groups respectively.
Results revealed that the average hospitalization cost for the On pump CABG
procedure was 590,522.47 baht, and 550,000.99 baht for the OPCAB. The
hospitalization cost for the On Pump CABG procedure with the low risk group was
460,348.16 baht, and 1,278,584.30 baht for the high risk group. The hospitalization
cost of the OPCAB with low risk group was 479,559.22 baht and 869,566.12 baht for
the high risk group. The effectiveness of both operative treatment procedures was
very similar, except that the quality of life score relating to general health status
postoperatively was higher in OPCAB patients. The qualitative results show that
elderly patients need more care and management postoperatively.
These findings suggest that the OPCAB procedure is safe for Thai patients,
involves lower hospitalization costs, especially for high risk patients, is more effective
and results in better quality of life postoperatively
