The effectiveness of education program on knowledge and self care behavior in patients with essential hypertension Prachathipat Hospital, Patumthanee province
Abstract:
This quasi-experimental research aimed to examine the effectiveness of an
education program on knowledge and self-care in patients with essential hypertension. The
sample consisted of patients with initial diagnosis of essential hypertension (3-month-1 year)
who attended hypertension clinic at Prachathipat hospital during January and March 2004.
A total of 74 subjects with the inclusion criteria, were divided into a comparison group
(n=37) and an experimental group (n=37). The experimental group received a 3-session
education program applying educative supportive nursing system for four weeks whereas the
comparison group recived the regular care from the hospital. Data were collected before and
after the intervention using the questionnaire, consisting of demographic information,
knowledge and self-care behavior related to hypertension. Data were analyzed by using
percentage, mean, standard deviation, paired t-test and independent t-test.
According to t-test, there were no significant differences on knowledge and self care
behavior among those in the experimental and the comparison group before the intervention.
After the intervention, the experimental group had higher scores on knowledge and self care
behavior than those before the intervention, and those in the comparison group with statistical
significance at p-value <0.001. There was significant difference on each dimension of
knowledge within the experimental group (p<0.001). Moreover, statistically significant
differences were observed in diet control, exercise, stress management after the intervention
within the experimental group. However no statistically significant difference was found in
medication taking within the experimental group before and after the intervention.
In addition, the mean differences on knowledge and self-care behavior reported by
the experimental group were statistically significant between before, on the one hand, and
after, and 1-month after the intervention, on the other. However, there was no statistically
significance different in knowledge and self-care behavior between after, and 1-month after
the intervention. Thus, the retention of knowledge and self-care behavior was not supported
in this study.
This study supports the effectiveness of the education program, applying Orem’s
educative supportive nursing system on knowledge and self-care behavior in patients with
initial diagnosis of essential hypertension. This findings suggest the application of the
education program to patients with hypertension or other chronic diseases in order to
enhance their self-care behavior.
