The effectiveness of applying the Roy adaptation model program on non-insulin dependent diabetes mellitus, Koh Samui hospital
Abstract:
Diabetes Mellitus is a chronic non-communicable disease with a
continuously increasing annual incidence rate. Compliance to a regimen is crucial
for patients in preventing life-threatening complications. This research aimed to
study the effectiveness of applying the Roy Adaptation Model (RAM) program on
Non-Insulin Dependent Diabetes Mellitus (NIDDM) patients at Koh Samui
Hospital.
This research was a quasi-experimental study with two-group pretestposttest
design. There were 61 patients, 31 patients in the experimental group and
30 patients in the comparison group. The experimental group received the RAM
program through a group process with 3 sessions at 2-week intervals followed by a
follow-up period of 4 weeks, while the comparison group received the regular
services from the hospital. The data were collected 3 times: pretest, posttest, and
posttest after one month by using an interview questionnaire. Percentage, mean,
standard deviation, independent sample t-test, paired t-test, one-way ANOVA, twoway
mixed design repeated measure ANOVA tests were used for statistical
analysis.
Data revealed that after implementing the program, the patients in the
experimental group had significantly and continuously increased their mean scores
of total and 4 modes of adaptation: physiological, self-concept, role-function, and
interdependence. These 4 mode practices included dietary control, taking
medication, exercise, foot care, and relaxation. In addition, the level of fasting
blood sugar of the experimental group was significantly lower at the end of the
program.
The findings showed that the RAM program with support from family and
group could positively improve the holistic adaptation of NIDDM patients.
Therefore, this program should be applied to patients in other hospitals as well as
patients with other chronic diseases
