Factors associated with diabetic retinopathy in type 2 diabetes

Abstract: This hospital based case control study was conducted at Diabetic Clinic, Vajira Hospital, Bangkok from May 26, 2003 to September 30, 2003 aims to determine the relationships among related factors and diabetic retinopathy in Type 2 diabetes patients. One hundred and thirty patients with diabetic retinopathy were the case group and 130 patients without diabetic retinopathy were the control group. They were matched at a ratio of 1:1 by gender and age (within 5 years). Multiple logistic regression analysis indicated that after adjusting for other variables in the model, only five variables were significantly associated with diabetic retinopathy: fasting plasma glucose (FPG) between 161 – 180 mg/dl and >180 mg/dl (OR = 3.82, 95 % CI 1.02 – 14.24 and OR = 5.98, 95 % CI 1.66 – 21.56), HbA1c > 9 % (OR = 4.09, 95 %CI 1.69 – 9.92), Hypertension (OR = 5.00, 95 % CI 2.64 – 9.46), Systolic blood pressure between 140-159 mmHg and ≥ 160 mmHg (OR = 3.22, 95 %CI 1.50 – 6.88 and OR = 9.98, 95 % CI 1.38 – 58.41) and BMI between 23-24.9,25-29 and ≥ 30 kg/m2 (OR = 0.20, 95 % CI 0.08 – 0.49, OR = 0.36,95% CI 0.16-0.83 and OR = 0.10,95% CI 0.03-0.32) respectively. Other variables, diastolic blood pressure, cholesterol, triglyceride, type of therapy, age at onset, duration of diabetes, smoking and alcohol consumption did not show significant association with diabetic retinopathy. This hospital based case–control study confirmed many risk factors in development of diabetic retinopathy, but failed to show any association between the other variables and diabetic retinopathy. This result can be utilized in a program of prevention and control, particularly in more aggressive management of glycemia and hypertension.

Abstract: การศึกษาแบบ hospital based case control study ซึ่งทำการศึกษาในในวิทยาลัยแพทย ศาสตร์และวชิระพยาบาล กรุงเทพมหานคร ตั้งแต่วันที่ 26 พฤษภาคม ถึง 30 กันยายน 2546 เพื่อ ศึกษาปัจจัยที่มีผลต่อการเกิดภาวะแทรกซ้อนที่จอประสาทตาในผู้ป่วยเบาหวานชนิดที่ 2 กลุ่มศึกษา ได้แก่ ผู้ป่วยเบาหวานที่มีภาวะแทรกซ้อนที่จอประสาทตา จำนวน 130 คน และกลุ่ม ควบคุมได้แก่ ผู้ป่วยเบาหวานที่ไม่มีภาวะแทรกซ้อนที่จอประสาทตา จำนวน 130 คน ซึ่งมีเพศ เดียวกันและอายุ แตกต่างกันไม่เกิน 5 ปี กับกลุ่มศึกษา ผลการศึกษาภายหลังจากการวิเคราะห์โดยใช้ multiple logistic regression เพื่อหาความ สัมพันธ์ระหว่างปัจจัยเสี่ยงกับการเกิดภาวะแทรกซ้อนที่จอประสาทตา โดยควบคุมตัวแปรกวนที่มี อิทธิพลต่อการเกิดโรค พบว่า ปัจจัยที่มีความสัมพันธ์กับการเกิดภาวะแทรกซ้อนที่จอประสาทตา ได้แก่ ระดับน้ำตาลในเลือด (FPG) ระหว่าง 161 – 180 มก.เดซิลิตร และมากกว่า 180 มก.เดซิลิตร (OR = 3.82, 95 % CI 1.02 – 14.24 และ OR = 5.98, 95 % CI 1.66 – 21.56) ระดับน้ำตาลในเลือด (HbA1c) มากกว่า 9 เปอร์เซ็นต์ (OR = 4.09, 95 %CI 1.69 – 9.92) ความดันโลหิตสูง (OR = 5.00, 95 % CI 2.64 – 9.46) ความดันโลหิตซิสโตลิคระหว่าง 140-159 มม.ปรอท และมากกว่าหรือเท่ากับ 160 มม.ปรอท (OR = 3.22, 95 % CI 1.50 – 6.88 และ OR = 9.98, 95 % CI 1.38 – 58.41 ) และ BMI ระหว่าง 23-24.9, 25-29 และ ≥ 30 กก./ม2 (OR = 0.20, 95 % CI 0.08 – 0.49, OR = 0.36, 95% CI 0.16-0.83 และ OR = 0.10, 95% CI 0.03-0.32 ) ตามลำดับ ส่วนความดันโลหิต ไดแแอสโตลิค คลอเลสเตอรอล ไตรกลีเซอร์ไรด์ ชนิดของการรักษา อายุที่เริ่มป่วย ระยะเวลาที่ป่วยเป็นเบาหวาน การสูบบุหรี่ การดื่มแแอลกอฮอล์ ไม่พบว่ามีความสัมพันธ์กับการ เกิดภาวะแทรกซ้อนที่จอประสาทตา จากผลการศึกษานี้แสดงปัจจัยที่ทำให้ผู้ป่วยเบาหวานชนิดที่ 2 เสี่ยงต่อการเกิดภาวะ แทรกซ้อนที่จอประสาทตา ข้อเสนอแนะ ควรดำเนินการป้องกันและควบคุมโรค เพื่อลดการเกิด ภาวะแทรกซ้อนในผู้ป่วยเบาหวาน โดยเฉพาะอย่างยิ่ง ระดับน้ำตาลในเลือด และความดันโลหิตสูง
