Effects of a supportive-eudcative nursing system on knowledge self care agency satisfaction on nursing care and clinical outcomes in patients with chronic kidney disease treated with hemodialysis
Abstract:
The purpose of this was to study the effects of a supportive - educative nursing system on knowledge, self-care agency, satisfaction on nursing care and clinical outcomes in patients with chronic kidney disease treated with homodialysis. Orem's theory of nursing system were used as a frame of reference. The quasi-experimental research two-group repeated measure pretest and posttest design was implemented. The sample consisted of the patients with chronic kidney disease treated with homedialysis in the hemodialysis center of two hospitals in Nakonsawan. The purposive sampling technique was used to recruit the 40 patients cemter of two hospitals in Nakonsawan. The purposive sampling technique was used to recruit the 40 patients treated with hemodialysis. The 20 patients were assigned into the experimental group who received the educative-supportive system 4 items about diet and fluids, medication, daily practice, practice homodialysis and emotions. The other 20 patients in the control group received the general nursing care. The instruments consisted of questionnaire covering; knowledge measurement; self-care agency measurement and satisfaction on nursing care measurement and clinical outcomes record of patients. The experiment program consisted of; lesson plans; self care manual; flipchart and video. The data were analyzed using t-test and chi-square test.After the experimental, it was found that patients have both knowledge and satisfaction on nursing care scores higher than before the study and it was significant at the .01 level. But self-care agency, interdialytic weight gain, level of potassium, level of albumin and follow up for hemodialysis was found no difference than the control group.According to the results of this study, nurses should adjust supportive-educative nursing system to be more effectiveness. And repeated studying patients with chronic kidney disease treated with hemodialysis to enhance self-care agency.
