Effect of Medagogy-Based Medication Literacy Enhancement on Medication Adherence Among Older Persons with Physical Multimorbidity

Abstract: Medication nonadherence is a leading problem in older persons with physical multimorbidity who have received polypharmacy, resulting in poor clinical outcomes. Therefore, enhancing medication literacy leading to medication adherence is essential for this group. This randomized controlled trial study aimed to examine the effect of Medagogy-based medication literacy enhancement on medication adherence among older persons with physical multimorbidity. One hundred participants were randomly selected and assigned into either a control group or an experimental group with 50 participants in each group. The control group received usual care, while the experimental group received the Medagogy-Based Medication Literacy Enhancement Program which consisted of exchanging information based on the PITS model (P-pathophysiology, I-indication, T-treatment, and S-specific) through transformative learning and the teach-back method. The program was divided into six sessions, implemented twice a week for three weeks. Medication adherence was measured using the Brief Medication Questionnaire (BMQ) at the 8th week of the program. The BMQ was tested for internal consistency reliability with a Cronbach’s alpha coefficient of 0.82. Data were analyzed using descriptive statistics and a t-test. The results were as follows: 1.The experimental group, after receiving the Medagogy-Based Medication Literacy Enhancement Program, had significantly higher mean scores for medication adherence than before receiving the program (p < ; .001). 2.The experimental group receiving the Medagogy-Based Medication Literacy Enhancement Program had significantly higher mean scores for medication adherence than the control group (p < ; .001). The results showed that enhancing medication literacy through transformative learning resulted in the improvement of medication adherence among older persons with physical multimorbidity. Therefore, health care providers can use the Medagogy-Based Medication Literacy Enhancement Program to improve medication adherence among this group.

Abstract: ความไม่ร่วมมือในการใช้ยาถือเป็นปัญหาที่สำคัญมากในผู้สูงอายุที่มีโรคเรื้อรังทางกายร่วมหลายโรคซึ่งได้รับยาหลายขนานส่งผลให้เกิดผลลัพธ์ด้านสุขภาพที่ไม่ดี ดังนั้นการเสริมสร้างความรอบรู้ด้านยาเพื่อนำไปสู่การเกิดความร่วมมือในการใช้ยาจึงมีความจำเป็นในผู้สูงอายุกลุ่มนี้ การศึกษานี้เป็นการทดลองแบบสุ่มและมีกลุ่มควบคุมมีวัตถุประสงค์เพื่อทดสอบผลของการเสริมสร้างความรอบรู้ด้านยาตามรูปแบบการสอนเพื่อให้ผู้ป่วยเป็นผู้เชี่ยวชาญด้านสุขภาพต่อความร่วมมือในการใช้ยาในผู้สูงอายุที่มีโรคเรื้อรังทางกายร่วมหลายโรคโดยคัดเลือกกลุ่มตัวอย่างแบบสุ่ม กลุ่มตัวอย่างทั้งหมด 100 ราย ได้รับการสุ่มเข้ากลุ่มควบคุม และกลุ่มทดลอง กลุ่มละ 50 รายเท่ากัน กลุ่มควบคุมได้รับการดูแลตามปกติ กลุ่มทดลองได้รับการเสริมสร้างความรอบรู้ด้านยา ประกอบด้วยการแลกเปลี่ยนข้อมูลโดยใช้กรอบของพิตส์ (PITS model) ซึ่งเป็นข้อมูลเกี่ยวกับพยาธิสภาพของโรค (P-pathophysiology) อาการ (I-indication) การรักษา (T-treatment ) และการรักษาเฉพาะ (S-specific)โดยใช้หลักการเรียนรู้สู่การเปลี่ยนแปลง และเทคนิคการสอนกลับ กิจกรรมจำนวน 6 ครั้ง สัปดาห์ละ 2 ครั้ง เป็นเวลา 3 สัปดาห์ และทำการเก็บรวบรวมข้อมูลความร่วมมือในการใช้ยาในสัปดาห์ที่ 8 โดยใช้แบบสอบถามความร่วมมือในการใช้ยาซึ่งได้รับการทดสอบความเชื่อมั่นแบบความสอดคล้องภายในได้ค่า สัมประสิทธิ์อัลฟาครอนบาค 0.82 การวิเคราะห์ข้อมูลใช้สถิติพรรณนา สถิติการทดสอบที (t-test) ได้ผลการศึกษาดังนี้ 1.กลุ่มทดลองที่ได้รับการเสริมสร้างความรอบรู้ด้านยามีคะแนนเฉลี่ยความร่วมมือในการใช้ยาสูงกว่าก่อนการทดลองอย่างมีนัยสำคัญทางสถิติ (p < ; .001) 2.กลุ่มทดลองที่ได้รับการเสริมสร้างความรอบรู้ด้านยามีคะแนนเฉลี่ยความร่วมมือในการใช้ยาสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p < ; .001) ผลการศึกษาพบว่าการเสริมสร้างความรอบรู้ด้านยาด้วยหลักการเรียนรู้สู่การเปลี่ยนแปลงส่งผลให้ผู้สูงอายุที่มีโรคเรื้อรังทางกายร่วมหลายโรคมีความร่วมมือในการใช้ยาดีขึ้น ดังนั้นบุคลากรสุขภาพสามารถนำโปรแกรมการเสริมสร้างความรอบรู้ด้านยาไปใช้เพื่อส่งเสริมความร่วมมือในผู้สูงอายุที่มีโรคเรื้อรังทางกายร่วมหลายโรค
