Effectiveness of fluoride dentifrice on preventing and arresting root caries : A meta-analysis

Abstract: Effectiveness of fluoride dentifrice on preventing and arresting root caries: A meta-analysis is a secondary data research on arresting and preventing root caries by using fluoride dentifrice. Since root caries is a common problem in older adults that is showing the increasing trend, it makes sense to investigate the preventive effect of fluoride on dental caries in this population group. Objective: To compare the preventive effects of fluoride dentifrice at different concentrations on new root caries lesions and to evaluate the effectiveness of fluoride dentifrice of different concentrations in arresting existing root caries lesions. Methods: The study complies PICO framework. Inclusion criteria were a randomized controlled trial study on root caries lesion in comparison of fluoride dentifrice to non-fluoride dentifrice conducted participants who be able to brush their own teeth, the article was in English or Thai language publication, and years of publication was between 1980 to February 2024. Articles were searched from 5 electronic databases: PubMed, EMBASE, Science Direct, Cochrane Library and ThaiJo. JADAD scale was used for risk of bias assessment. The meta-analysis was done in both fixed and random effect models. Results: There were 1,638 studies from 5 electronic databases under inclusion and exclusion criteria. After fully reviewing, only 3 studies left for the meta-analysis method. 2,749 lesions were included in the studies. The analysis showed a superior effect of 5,000 ppm fluoride dentifrice on arrested root caries lesions about 2.17-2.18 times higher than those containing less than 5,000 ppm fluoride (95%CI : 1.97,2.40). Limitation was data analysis difficulties because of a small number of studies included in the meta-analysis.

Abstract: ประสิทธิผลของยาสีฟันที่มีส่วนผสมของฟลูออไรด์ในการป้องกันและหยุดการลุกลามของรากฟันผุ : การวิเคราะห์อภิมานเป็นงานวิจัยขั้นทุติยภูมิที่ทำการวิเคราะห์อภิมานข้อมูลจากงานวิจัยที่ศึกษาเรื่องการป้องกันและหยุดการลุกลามของรากฟันผุหลังใช้ยาสีฟันที่มีส่วนผสมของฟลูออไรด์ เนื่องจากรากฟันผุเป็นปัญหาที่มักพบในกลุ่มผู้สูงอายุ วัตถุประสงค์งานวิจัยคือ เพื่อเปรียบเทียบประสิทธิผลของยาสีฟันที่มีส่วนผสมของฟลูออไรด์ในการป้องกันการเกิดรากฟันผุขึ้นใหม่ และค้นหาความเข้มข้นของฟลูออไรด์ในยาสีฟันที่จะสามารถหยุดการลุกลามของรากฟันผุได้ ขั้นตอนการดำเนินงานวิจัยมีดังนี้ ภายใต้การใช้โครงสร้าง PICO ในการกำหนดคำถามงานวิจัย ได้ทำการคัดเลือกงานวิจัยที่คัดเข้ามาเพื่อใช้ในขั้นตอนวิเคราะห์อภิมาน คือ งานวิจัยแบบสุ่มและมีกลุ่มควบคุมที่ทำการศึกษาเปรียบเทียบระหว่างกลุ่มที่ใช้ยาสีฟันที่มีส่วนผสมของฟลูออไรด์กับยาสีฟันที่ไม่มีส่วนผสมของฟลูออไรด์ หรือยาสีฟันที่มีส่วนผสมของฟลูออไรด์ที่ความเข้มข้นต่างกันในอาสาสมัครที่สามารถแปรงฟันได้ด้วยตนเอง โดยงานวิจัยที่ถูกคัดเข้ามาในการวิเคราะห์อภิมานทั้งภาษาอังกฤษและไทย ที่ทำการศึกษาค้นคว้าระหว่างปี คศ. 1980 - เดือนกุมภาพันธ์ ค.ศ. 2024 ทำการสืบค้นข้อมูลจากฐานข้อมูลอิเล็กทรอนิกส์ 5 แห่ง คือ PubMed, EMBASE, Science Direct,Cochrane Library และ ThaiJo หลังจากนั้นทำการประเมินคุณภาพงานวิจัยให้อยู่ในระดับสูงโดยใช้มาตรวัด Jadad Scale ผลการศึกษาพบว่า จากฐานข้อมูลอิเล็กทรอนิกส์ทั้ง 5 แห่ง พบว่ามีงานวิจัยที่ผ่านเกณฑ์คัดเข้าทั้งหมด 1,638 ชิ้น และหลังจากทำการตรวจสอบแบบเต็มพบว่ามีเพียง 3 ชิ้นที่ผ่านเกณฑ์คัดเข้าเพื่อใช้ในขั้นตอนวิเคราะห์อภิมาน จากงานวิจัย 3 ชิ้นพบว่า มีรอยโรครากฟันผุทั้งหมด 2,749 รอยโรค หลังทำการวิเคราะห์อภิมานพบว่า การใช้ยาสีฟันที่มีส่วนผสมของฟลูออไรด์ ที่ความเข้มขน 5,000 ส่วนในล้านส่วน มีผลในการหยุดการลุกลามของรากฟันผุได้มากกว่ายาสีฟันที่มีความเข้มข้นต่ำว่า 5,000 ส่วนในล้านส่วน อยู่ 2.17-2.18 เท่า (95%CI : 1.97,2.40) และข้อจำกัดของานวิจัยนี้คืองานวิจัยที่ผ่านเกณฑ์การคัดเข้าและสามารถใช้ในการวิเคราะห์อภิมานได้นั้นมีจำนวนน้อย
