Abstract:
เข่าเทียมระบบหลายจุดศูนย์กลางได้ถูกนำไปใช้กันอย่างแพร่หลาย สำหรับผลิตขาเทียมชนิดเหนือเข่า เพราะส่วนประกอบทางกลของเข่าเทียมชนิดนี้สามารถเพิ่มความมั่นคงในช่วงเวลาการยืน และส่งผลคล้ายกับการกระดกข้อเท้าขึ้นเล็กน้อยในช่วงเวลาการแกว่ง แต่ถึงกระนั้นผู้พิการยังคงไม่ค่อยพอใจกับระยะการกวาดปลายเท้าที่ได้ ถึงแม้ว่าจะใช้เข่าเทียมระบบหลายจุดศูนย์กลางก็ตาม ฉะนั้นวิทยานิพนธ์เล่มนี้จึงได้ตั้งสมมติฐานว่า การกระดกข้อเท้าที่สอดคล้องกับช่วงเวลาการงอเข่า จะสามารถช่วยเพิ่มระยะการกวาดปลายเท้าให้เพิ่มขึ้น และได้ผลลัพธ์มากกว่าการใช้ขาเทียมกับระบบเข่าหลายจุดศูนย์กลาง โดยสมมติฐานนี้จะถูกประเมินจากการตรวจสอบระยะการกวาดปลายเท้าเหนือพื้น ด้วยการประมวลผลทางกลศาสตร์การเคลื่อนไหว ผลการทดลองที่ได้แสดงให้เห็นว่า การกระดกข้อเท้าที่สอดคล้องกับการงอเข่ามีค่าการกวาดปลายเท้า (9.15 ซม.) ในช่วงกึ่งกลางของช่วงระยะการแกว่ง สูงกว่าขาเทียมที่ใช้ระบบเข่าหลายจุดศูนย์กลาง (5.60 ซม.) เพราะฉะนั้นการกระดกข้อเท้าที่สอดคล้องกับการงอเข่าจึงสมควรที่จะถูกนำมาพิจารณา สำหรับการออกแบบขาเทียมของผู้พิการให้ดีขึ้น
Abstract:
Polycentric knees are frequently used in transfemoral prostheses because they provide added stability in stance phase and relative ankle dorsiflexion in swing phase. Despite this relative ankle dorsiflexion, patients still complain of inadequate toe clearance even with polycentric knees. This thesis hypothesizes that active ankle dorsiflexion via a knee-ankle coupling provides more toe clearance than that provided by polycentric knees during mid-swing the most critical phase of toe clearance. This hypothesis is evaluated by examining toe clearance of kinematic knee models in prostheses and processing existing data on amputee gait. A knee with active ankle dorsiflexion achieved larger toe clearance (9.15 cm) than the polycentric knee (5.60 cm) during mid-swing. Knee designs with ankle dorsiflexion should be reconsidered for improved toe clearance in the gait of amputees.
