The study of efficacy and safety of a novel hair tonic

Abstract: Androgenetic alopecia is a common problem that may affect both males and females in all age groups. The availability of a topical treatment which appears to reverse the problem of androgenetic alopecia has encouraged many scientists to seek medical help. Hair tonic has long been presented for the specific purposes of curing baldness. The present study aims to develop a novel hair tonic and study the safety in animals and the efficacy and safety in man. The studies of safety of the hair tonic in animals were conducted to assess skin irritation based on the methods described by Uttley&van Abbe as well as that established by Draize. In addition, the skin sensitization test was performed in guinea pig according to Buehlers method. An open 24-week study was conducted to evaluate the safety and efficacy of hair tonic in subjects aged 24 to 50 years with androgenetic alopecia categorized according to Hamilton and Ludwig classifications. Of 14 males and 3 females enrolled in the study, 10 males and 1 female completed the study. They were instructed to apply 1 ml of the hair tonic, twice daily at 12 hr apart, directly on the bald area and massaged the scalp for 5 minutes each time during the whole period. Efficacy were determined every 2 weeks by objective nonvellus hair count, number of hair shedding per day, the diameter of bald area and by subjective evaluations of both the investigator and subjects at the end of the study. A clear solution of hair tonic with pleasant smell of rosemary oil was classified as mild irritating substance and a weak sensitizer after animal testings. At the end of the efficacy study, the average nonvellus hair count/cm(2) increased from a baseline mean of 90 to 120. The increase was highly significant (P=0.0059). A decrease from a baseline in hair shedding per day and the diameter of bald area were noted in 90% and 12.5% of subjects, respectively. According to investigators subjective evaluation at week 24, 27% of subjects had minimal new hair growth. Eighty-one percents of subjects reported that they had minimal new hair growth. The side effect of the hair tonic was rare and not serious and manifested as mild itching and fine vesicle formation at the site of application. The novel hair tonic developed in this study appears to be safe and effective in prevention and retarding progression of androgenetic alopecia.

Abstract: ศีรษะล้านเป็นปัญหาทั่วไปซึ่งพบได้ทั้งในบุรุษและสตรี ในทุกๆ ช่วงอายุ การที่มีการผลิตผลิตภัณฑ์บำรุงผมหลายชนิด ซึ่งมีแนวโน้มที่จะมีประสิทธิภาพในการช่วยแก้ปัญหาของผู้ที่มี ศีรษะล้านชนิด Androgenetic alopecia ออกมา เป็น แรงจูงใจให้นักวิจัยมีความพยายามที่จะคิดค้นหาตัวยาและ สารใหม่ๆ ที่มีประสิทธิภาพสูงกว่าสารที่มีอยู่เดิมเพื่อใช้ ในการรักษา ผลิตภัณฑ์บำรุงผมได้มีการใช้กันมานานแล้ว เพื่อช่วยในการรักษาอาการศีรษะล้านโดยเฉพาะ การศึกษานี้ มีจุดประสงค์ที่จะพัฒนาผลิตภัณฑ์บำรุงผมชนิดใหม่ และทำการ ศึกษาความปลอดภัยของการใช้ผลิตภัณฑ์ในสัตว์ทดลอง ทำการ ศึกษาถึงประสิทธิภาพและความปลอดภัยของผลิตภัณฑ์ในคน ได้ทำการศึกษาความปลอดภัยในสัตว์ทดลองโดยทดสอบ การระคายเคืองโดยใช้วิธีของ Uttley และ van Abbe และ วิธีของ Draize อีกทั้งได้ทดสอบการก่อให้เกิดอาการแพ้ทาง ผิวหนังโดยวิธีของ Buehler โดยใช้หนูตะเภา ส่วนการศึกษา ประสิทธิภาพและความปลอดภัยของผลิตภัณฑ์บำรุงผมในคนเป็น การศึกษาแบบเปิด (open study) ใช้ระยะเวลาในการศึกษา 24 สัปดาห์ โดยเลือกอาสาสมัครอายุระหว่าง 24-50 ปี ที่มี ปัญหาศีรษะล้านแบบ Androgenetic alopecia ตามการแบ่ง ของ Hamilton และ Ludwig มีอาสาสมัครเข้าร่วม 17 คน แบ่งเป็นชาย 14 คน และหญิง 3 คน เมื่อสิ้นสุดการทดลอง จำนวนอาสาสมัครชายที่ยังร่วมโครงการเหลืออยู่ 10 คน และ หญิง 1 คน โดยอาสาสมัครต้องทาผลิตภัณฑ์บำรุงผมบริเวณศีรษะ ที่ผมบางครั้งละ 1 มิลลิลิตร วันละ 2 ครั้ง ห่างกัน 12 ชั่วโมง โดยนวดบริเวณศีรษะครั้งละ 5 นาที ตลอดระยะเวลาการศึกษา ได้ประเมินประสิทธิภาพทุก 2 สัปดาห์โดยการนับจำนวนเส้นผม ต่อพื้นที่ 1 ตารางเซ็นติเมตร นับจำนวนเส้นผมที่ร่วงในแต่ละวัน และวัดเส้นผ่าศูนย์กลางของบริเวณศีรษะที่ผมบาง นอกจากนี้ ผู้ทำวิจัยและอาสาสมัครยังทำการประเมินประสิทธิภาพของ ผลิตภัณฑ์บำรุงผมโดยการประเมินตามความรู้สึกหลังจากทา ผลิตภัณฑ์นี้ครบตามระยะเวลา ผลิตภัณฑ์บำรุงผมที่พัฒนาขึ้นมาใหม่ มีลักษณะสีเหลืองอ่อน ใส และมีกลิ่นหอมของโรสแมรี่ออยล์ เมื่อนำไปทดสอบในสัตว์ ทดลอง พบว่าทำให้เกิดการระคายเคืองต่อผิวหนังเพียงเล็กน้อย และไม่ทำให้เกิดการแพ้ เมื่อทดสอบประสิทธิภาพของผลิตภัณฑ์ บำรุงผมในคนโดยศึกษาครบ 24 สัปดาห์ พบว่า จำนวนเส้นผม เฉลี่ยต่อพื้นที่ 1 ตารางเซ็นติเมตร เพิ่มขึ้นจาก 90 เส้น ที่เวลาก่อนใช้ผลิตภัณฑ์บำรุงผม เป็น 120 เส้น ซึ่งเป็นการ เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (P=0.0059) นอกจากนี้ยัง พบว่าอาสาสมัคร 90% มีจำนวนเส้นผมที่ร่วงในแต่ละวันลด น้อยลง และมีอาสาสมัครจำนวน 12.5% มีพื้นที่บริเวณศีรษะ ที่ผมบางเล็กลง ผู้วิจัยได้ทำการประเมินประสิทธิภาพของ ผลิตภัณฑ์บำรุงผมจากการสังเกตภาพรวมพบว่า 27% ของ อาสาสมัครมีผมขึ้นใหม่เพียงเล็กน้อย ส่วนอาสาสมัคร 81% ได้รายงานว่ามีผมขึ้นเล็กน้อย การใช้ผลิตภัณฑ์บำรุงผมนี้ ไม่ก่อให้เกิดผลข้างเคียงที่เป็นปัญหารุนแรง เพียงแต่พบ อาการคันศีรษะและมีเม็ดเล็กๆ ขึ้นตรงบริเวณที่ใช้ผลิตภัณฑ์ ในอาสาสมัครบางรายเท่านั้น ดังนั้นจึงสรุปได้ว่าผลิตภัณฑ์บำรุงผมที่พัฒนาขึ้นใหม่ มีความปลอดภัยในการใช้ และมีประสิทธิภาพในการช่วย ป้องกันและยับยั้งไม่ให้ศีรษะล้านมากขึ้น และลดอาการ ผมร่วงในผู้ที่มีปัญหาศีรษะล้านแบบ Androgenetic alopecia
