Factors associated with medication adherence of patients with type 2 diabetes mellitus in Khumaung district Burirum province Thailand
Abstract:
The present study was a case-control study which investigated different
factors affecting medication adherence of patients with Type 2 diabetes who sought
treatment at the Diabetes Clinic of Khumaung Hospital or one of the ten community
health centers in Khumaung District, Burirum Province. Data collection took place
between October 2008 and January 2009. The total number of subjects, who were
selected by means of purposive sampling based on the OPD cards, was 254. The
subjects were divided into two groups. One hundred and twenty-seven subjects in the
case group were those who had medication adherence, while the other 127 subjects in
the control group were those with medication non-adherence. Data were collected by
means of interviews based on a set of questionnaires, and data were analyzed using
descriptive statistics and Pearsons product moment correlation to determine the
relationship between different factors and medication adherence.
The study findings showed that gender was associated with medication
adherence as female patients were more likely to have medication adherence than male
patients, with statistical significance (95% CI = 1.134 3.759). An increase in types of
diabetic medication to two or more types, reduced medication adherence when
compared to receiving only one type of medication, with statistical significance (95%
CI = 0.116 0.401). If the number of pills prescribed by the doctor was more than
three pills it reduced medication adherence when compared to those receiving less
than three pills, with statistical significance (95% CI = 0.283 0.957). It knowledge
scores lower than moderate reduced medication adherence with a statistical
significance (95% CI = 0.066 0.205), The perception scores lower than moderate
reduced medication adherence, with a statistical significance (95% CI = 0.010
0.112), and the social support scores lower than moderate reduced medication
adherence, with a statistical significance (95% CI = 0.078 0.753).
Based on the findings of this study, it is recommended that nurses should
find ways to better encourage patients to have more medication adherence. In addition,
when caring for patients with Type 2 diabetes, nurses should work with the
multidisciplinary team to enable the patients to adjust their other behaviors such as
stimulating them to exercise more and to more strictly control their diets so as to be
better able to control their blood sugar level and reduce the amount of medication
intake. Experimental studies, therefore, should be conducted to more effectively
promote medication adherence among patients with Type 2 diabetes
