The development of a referral system for chronic disease patients from Pramongkutklao Hospital to health centers of Bangkok metropolitan administration.
Abstract:
This study developed a referral system for chronic disease patient from Pramongkutklao
Hospital to Health Centers of Bangkok Metropolitan Administration. The study applied the following
concepts and developmental processes. 1) Analyzing the present referral systems of Pramongkutklao
Hospital and health centers of Bangkok Metropolitan Administration by applying the concepts and
theories in regards to a referral system and the examples of referral systems developed in various
health centers. This was in order to develop a conceptual framework for carrying the analysis and
evaluation. 2) Evaluating the feasibility and conditions of developing the referral system by bolting
at the readiness of the policy, structure, management system, and resources especially human
resources. 3) Drafting a new model of the referral system by utilizing the outcomes of steps 1 and 2
as the lessons and conceptual framework. 4) Evaluating the referral model by using 3 groups of
personnel: administrators, practitioners, and academics in order to get opinions and suggestions for
developing the referral models that it can be put into practice effectively.
The result of the study showed that there was a need to develop a referral system in order
to improve patients quality of life. Both health organizations are ready to work cooperatively in
regards to policy, personnel, and management system. The referral model developed composed of 3
important components: referral structure and system, whereby a new unit must be established in the
health service unit in order to collaborate and implement the activities; human resource preparation,
whereby the personnel need to be enhanced in regards to concepts, techniques, and methods; and
lastly, resource management and all action plans. This newly developed referral system was agreed
upon by all related personnel, especially by the administrators and the practitioners.
The result of this study is directly useful for Pramongkutklao Hospital in establishing a
referral system for chronic disease patients that provides benefits for the patients, as well as
increasing the service efficiency of Pramongkutklao Hospital and associated health centers. This new
referral system will also be a good example of mutual cooperation and assistance between two
organizations that can health, improve quality of life of patients, and their families. This type of
developed referral system should be applied in various types of health service organizations.
