Case study of effect of cognitive behavioral therapy program on insight of schizophrenic patients
Abstract:
This study aimed to study the effect of cognitive behavioral therapy (CBT)
program on pre-program and post-program insight of schizophrenic patients. Six
patients with first- and second-episode schizophrenia were recruited in this study.
They were treated in the psychiatric ward, Jitavej Khonkaen Rajanakarindra Hospital
where they had been diagnosed as schizophrenic. The patients received a CBT
program for schizophrenic patients which was developed by the researcher. This CBT
program was modified from the conceptual framework of Fowler, Garety and Kuipers.
It consisted of four stages, containing twelve sessions and each session was 15-60
minutes. The study measurements included patient’s demographic survey, Standard
Progressive Matrices (Intelligence test) and Insight Rating Scale. Data was analyzed
by comparing between pre- and post-test of insight level.
The result indicated that insight level of the three schizophrenic patients who
received the complete cognitive behavioral therapy program had increased after the
therapy and at the follow-up period. They were aware of illness, complied with
medication, adhered to the treatment process and relabelled psychosis experiences
such as auditory hallucination or delusion as pathology. At the same time, the insight
level of the three patients who dropped out was low. They did not accept the
medication and lacked insight. After dropping out, they contacted the hospital
irregularly and one of the three patients had an illness relapse and was readmitted
within three months after discharge
