Alcohol consumption related to essential hypertension in Nan province
Abstract:
This study was a hospital-based case-control study, which was set up to
determine the relationship between alcohol consumption and essential hypertension.
Data were collected by using structured questionnaires and some data was copied
from the patients, medical record (OPD card). Subjects were 151 hypertensive patients
who received treatment and 151 healthy subjects who came for an annual physical
check-up or were general patients at the Out Patient department of Pua hospital, Nan
Province, Thailand, during October 2002 to February 2003. For each case, a control
was selected and matched by sex and age (within 5 years).
Multiple logistic regression analysis of alcohol consumption related to
essential hypertension by controlling conventional confounders showed that the
factors significantly related to essential hypertension were : current drinkers
(ORadj=2.27, 95%CI=1.13-4.56), duration of alcohol drinking of 11-20 years
(ORadj=2.23, 95%CI=1.01-4.91), drinkers duration of > 21 years (ORadj=2.47,
95%CI=1.05-5.84), drinkers who drank 3-4 days / week (ORadj=3.38, 95%CI=1.06-10.79)
and drinkers who drank > 281 gm / week (ORadj=8.87, 95%CI=2.34-33.59). These
factors gave a higher risk of essential hypertension than non drinkers. This model was
adjusted for age, Body mass index (BMI), history of essential hypertension in family,
salty food, total fiber food, Type A behavior (constant, rushed, activity, aggression
and want of accomplishment), smoking and educational level.
For the effect of combination between frequency and quantity of alcohol
consumption, drinkers who spent 1 or 2 days per week on a quantity of > 280 gm / week
(ORadj=5.78, 95%CI=1.11-31.05) and drank 3 or 4 days per week on quantity 281
gm or more (ORadj=15.14, 95%CI=2.18-105.07) had higher risk of essential
hypertension than non drinkers. Therefore, the campaign to reduce quantity alcohol
consumption is expected to have an effect on reduction of essential hypertension
incidence. Excessive alcohol consumption reduction must be considered and promoted
by public health organizations or government organizations or non-government
organizations.
