Study of knowledge, practice, problems and barriers in discharge planning of professional nurses in Nopparatrajathanee hospital
Abstract:
This study was a descriptive research which aimed to study knowledge,
practice, problems and barriers in discharge planning of professional nurses in
Nopparatrajathanee hospital. The sample were 94 professional nurses who provided
direct care to the patients in the ward. Data were collected by using questionnaires
developed by the investigator and were analyzed by descriptive statistics.
The results showed that the majority of the sample were female (98.94%), aged
between 36-45 years (58.51%) had never trained in discharge planning. The
Knowledge score in discharge planning of professional nurses was at the moderate
level almost to high level (X 16.90,SD 1.74) and their practice was in the high level
(X 3.00,SD0.55). When considering each step of the discharge planning process, it
was found that implementing the discharge plan was the step that performed the
most (3.22,SD0.60) followed by the step of assessment (X 3.20,SD0.49), diagnosis
(X 3.05,SD0.70), planning (X 2.76, SD0.67) and at least in evaluation (X
2.56,SD0.78).
In relation to problems and barriers in discharge planning, professional nurses
stated that the most important were responsible for a lot of work (37.23%), lack of
cooperation from relatives and families (30.85%), the activities for discharge
planning
were not systemic and continual (30.85%) and lack of coordination with a
multidisciplinary team (20.21%).
The recommendations for improving discharge planning by professional nurses
were that there should be an available manual to serve as a guideline for discharge
planning (45.74%), there should be continual training (41.49%), there should be a
specialty team for discharge planning (30.85%) and there should be a clearly
systemic document (23.40%).
In terms of future research, there should be further qualitative studies to explore
more of the problems and barriers of discharge planning practices as well as the
ways to improve discharge practices of nurses.
