Abstract: การศึกษาวิจัยครั้งนี้ มีวัตถุประสงค์เพื่อสร้างมาตรฐานเชิงกระบวนการ มาตรฐานเชิงเนื้อหา และมาตรฐานเชิงผลลัพธ์ ของการพยาบาลสำหรับเตรียมญาติผู้ดูแลเพื่อการดูแลผู้ป่วยที่มีแผลกดทับที่บ้าน ดำเนินการวิจัยโดยร่างมาตรฐานการพยาบาลตามขั้นตอนของเมสัน จากข้อมูลหลักฐานเชิงประจักษ์ ร่วมกับผลการสัมภาษณ์เชิงลึกญาติผู้ดูแล จำนวน 5 คน และตรวจสอบความเที่ยงตรงด้านเนื้อหา โดยผู้เชี่ยวชาญจำนวน 23 คน โดยใช้เทคนิคเดลฟายจำนวน 3 รอบการวิเคราะห์ข้อมูลใช้การแจกแจงความถี่ ร้อยละ มัธยฐาน และพิสัยระหว่างวอร์ไทล์ผลการวิจัย ได้มาตรฐานกรพยาบาลสำหรับเตรียมญาติผู้ดูแลผู้ป่วยที่มีแผลกดทับที่บ้าน จำนวน 8 มาตรฐาน ประกอบด้วย มาตรฐานเชิงกระบวนการ จำนวน 64 ข้อ มาตรฐานเชิงเนื้อหา จำนวน 16 ข้อ และมาตรฐานเชิงผลลัพธ์ จำนวน 23 ข้อ โดยมีรายละเอียดดังนี้ มาตรฐานที่ 1 การเตรียมความพร้อมของพยาบาลวิชาชีพก่อนเตรียมญาติผู้ดูแล มาตรฐานที่ 2 การสร้างสัมพันธภาพที่ดีกับผู้ป่วยที่มีแผลกดทับและครอบครัว มาตรฐานที่ 3 การประเมินปัญหาและความต้องการของผู้ป่วยที่มีแผลกดทับ มาตรฐานที่ 4 การเตรียมความพร้อมของญาติผู้ดูแลก่อนการเรียนรู้และสภาพแวดล้อมที่เอื้อต่อการเรียนรู้ มาตรฐานที่ 5 การส่งเสริมให้ญาติผู้ดูแลมีความรู้ความเข้าใจเกี่ยวกับการดูแลผู้ป่วยที่มีแผลกดทับแบบองค์รวม มาตรฐานที่ 6 การส่งเสริมให้ญาติผู้ดูแลมีทักษะที่จำเป็นเกี่ยวกับการดูและแผลกดทับ มาตรฐานที่ 7 การสร้างและพัฒนาความร่วมมือกับแหล่งเครือข่ายสนับสนุนดูแลสุขภาพที่บ้าน และมาตรฐานที่ 8 การประเมินผลการพยาบาลตามมาตรฐานการพยาบาลสำหรับเตรียมญาติผู้ดูแลเพื่อการดูแลผู้ป่วยที่มีแผลกดทับที่บ้านจากนั้นไปศึกษาถึงความเป็นไปได้ในการนำมาตรฐานการพยาบาลไปใช้ในสถานการณ์จริง โดยศึกษาความคิดเห็นของพยาบาลวิชาชีพ ผู้ปฏิบัติงานเกี่ยวกับการดูแลผู้ป่วยที่มีแผลกดทับที่บ้าน จำนวน 30 คน ผลการศึกษาพบว่า พยาบาลวิชาชีพส่วนใหญ่เห็นด้วยว่ามาตรฐานการพยาบาลโดยรวมสามารถนำไปใช้แนวทางในการปฏิบัติได้ในสถานการณ์จริง

Abstract: The purpose of this research was to construct nursing standard focusing on processes, contents and outcomes of family caregiver preparation for home health care in patients with pressure sore. Evidence-base practice and Mason,s method (1994) were used as the guidelines and framework for this standard. Content validity of this standard was verified by 23 experts utilizing 3 round of Delphi technique. The agreement of the experts opinion was analyzed using median and inter-quartile range. Finally, the 30 home health care nurse opinions of feasibility to apply the nursing standard in practice were explored by using questionnaire.The results revealed that this nursing standard composed of eight elements including 64 items of process, 16 items of content and 23 items of outcome standard were as follows: Standard 1) Self preparation for nurse to prepare caregiver., Standard 2) Establishment of interrelationship among patients and family., Standard 3)The patients assessment and identification of leaning need in caregiver., Standard 4) The readiness preparation of caregiver and environment., Standard 5) The knowledge distribution of pressure sore care., Standard 7) Establishment and development of community network collaboration., Standard 8) The evaluation of overall nursing standards. Finally, the majority of 30 home health care nurse opinion accepted that this nursing standards were applicable to real situation.
